阴部神经阻滞麻醉在RPH治疗轻中度内痔中的效果分析

摘　　要：目的探讨阴部神经阻滞麻醉在自动痔疮套扎术(RPH)治疗轻中度内痔中的应用效果。方法 50例轻中度内痔患者,按照盲选法随机分为对照组和观察组,各25例。对照组在腰椎麻醉下进行RPH治疗,观察组在阴部神经阻滞麻醉下进行RPH治疗。比较两组患者疼痛抑制效果、术后并发症发生情况及麻醉生效时间。结果两组患者术中及术后2、24 h的视觉模拟评分法(VAS)评分对比差异无统计学意义(P>0.05)。观察组并发症发生率16.00%低于对照组的44.00%,差异有统计学意义(P<0.05)。观察组麻醉生效时间与对照组对比差异无统计学意义(P>0.05)。结论在阴部神经阻滞麻醉下进行RPH治疗轻中度内痔的效果理想,可改善传统腰椎麻醉术后并发症发生率高的问题,保障耐受力差、年龄大患者的预后恢复。Objective To discuss the practical effect of pudendal nerve block anesthesia in the treatment of mild to moderate hemorrhoids with ruiyun procedure for hemorrhoids(RPH). Methods A total of 50 patients with mild to moderate hemorrhoids were randomly divided into control group and observation group, with 25 cases in each group. The control group received RPH under lumbar spinal anesthesia, and the observation group received RPH under pudendal nerve block anesthesia. The pain suppression effect, occurrence of postoperative complications and the onset time of anesthesia were compared between the two groups. Results There was no statistically significant difference in visual analogue scale(VAS) score during operation, 2 and 24 h after operation between the two groups(P>0.05). The incidence of complications 16.00% of the observation group was lower than 44.00% of the control group, and the difference was statistically significant(P<0.05). The onset time of anesthesia of the observation group had no statistically significant difference compared with that of the control group(P>0.05). Conclusion The effect of RPH under pudendal nerve block anesthesia is ideal in the treatment of mild to moderate hemorrhoids. It can improve the high incidence of complications after traditional lumbar anesthesia and ensure the prognosis of patients with poor tolerance and older age.
