摘  要:目的评价颈椎多体位磁共振成像(MRI)对颈椎病患者的诊断价值。方法选取30例颈椎病患者作为研究对象,对其分次进行前屈、后伸及中间位等多体位行磁共振T2WI矢状位扫描。统计患者后伸位、前屈位颈椎C4~5椎间隙水平椎管硬膜囊间距(DSD)减少情况,测量颈椎中立位、前屈位及后伸位状态下C4~5椎间隙水平DSD。结果 30例患者中后伸位颈椎C4~5椎间隙水平DSD减少21例,前屈位颈椎C4~5椎间隙水平DSD减少9例。患者颈椎中立位、前屈位及后伸位状态下C4~5椎间隙水平DSD分别为(8.3±1.2)、(8.6±1.6)、(7.8±1.3)mm,颈椎中立位状态下C4~5椎间隙水平DSD与前屈位比较差异无统计学意义(P>0.05);前屈位状态下C4~5椎间隙水平DSD大于后伸位,差异有统计学意义(P<0.05)。结论通过对颈椎病患者行多体位磁共振成像,有助于发现潜在的颈椎椎管变化情况,从而为临床医生的诊治提供更有价值的指导。Objective To evaluate the diagnostic value of cervical multi-position magnetic resonance imaging(MRI) for cervical spondylopathy. Methods 30 patients with cervical spondylopathy were selected as the research subjects, and they were scanned in multiple positions such as flexion, extension, and mid-position with magnetic resonance T2 WI. The reduction of spinal dural sac distance(DSD) at the level of C4-5 intervertebral space in extension position and flexion position was statistically analyzed, and the level of DSD of C4-5 intervertebral space was measured in neutral position, flexion position and extension position. Results Among the 30 patients, the DSD of cervical C4-5 intervertebral space decreased in 21 cases at extension position and in 9 cases at flexion position. The DSD of the C4-5 intervertebral space level of the patient in the neutral, flexion and extension positions of the cervical spine were(8.3±1.2),(8.6±1.6) and(7.8±1.3) mm, respectively. There was no statistically significant difference in DSD of C4-5 intervertebral space between neutral position and flexion position(P>0.05). The C4-5 intervertebral space level DSD in the flexion position was greater than that in the extension position, and the difference was statistically significant(P<0.05). Conclusion Multi-position magnetic resonance imaging for patients with cervical spondylosis can help discover hidden changes in the cervical spinal canal, thereby providing more valuable guidance for clinicians in diagnosis and treatment.
