奥曲肽联合奥美拉唑对上消化道出血的疗效探讨

摘　　要：目的分析奥曲肽联合奥美拉唑在上消化道出血患者中的治疗效果。方法将该院2016年4月—2018年5月期间83例上消化道出血患者纳入到该次调查研究中,双盲法纳入分组,观察组患者采用奥曲肽+奥美拉唑疗法,对照组患者给予奥美拉唑,对比两组患者临床效果。结果观察组患者止血时间(26.82±5.92)h,对照组止血时间(33.52±7.04)h,观察组患者止血时间明显低于对照组,差异有统计学意义(P<0.05);观察组患者症状消退时间(3.63±0.89)h,对照组症状消退时间(5.69±1.27)h,观察组患者症状消退时间明显低于对照组,差异有统计学意义(P<0.05);观察组患者住院时间(7.93±9.75)h,对照组住院时间(11.82±6.07)h,观察组患者症状消退时间明显低于对照组,差异有统计学意义(t=11.268、6.921、8.221,P<0.05);观察组止血率(95.12%vs 76.19%)、治疗总有效率(92.68%vs 73.81%)均高于对照组,差异有统计学意义(χ^(2)=7.204、5.347,P=0.017、0.015);观察组与对照组患者用药期间不良反应发生率比较(7.32%vs 9.52%),差异无统计学意义(χ^(2)=0.635,P=0.289)。结论奥曲肽联合奥美拉唑治疗上消化道出血,临床总体效果良好,能够显著提高疗效,减少出血量,用药方案安全有效,值得推广。Objective To analyze the therapeutic effect of octreotide combined with omeprazole in patients with upper gastrointestinal bleeding.Methods A total of 83 patients with upper gastrointestinal bleeding from April 2016 to May 2018 in the hospital were included in this investigation.Double-blind method was included in the groups.Patients in the observation group received octreotide+omeprazole therapy,and the control group was given omeprazole to compare the clinical effects of the two groups.Results The hemostasis time of patients in the observation group was(26.82±5.92)h,and the time of hemostasis in the control group was(33.52±7.04)h.The hemostasis time of patients in the observation group was significantly lower than that of the control group,the difference was statistically significant(P<0.05),the time of symptom resolution in the control group was(5.69±1.27)h.The time of symptom resolution in the observation group was(3.63±0.89)h.The symptom resolution time of the observation group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05);the hospital stay of the observation group was(7.93±9.75)h,and the hospital stay in the control group was(11.82±6.07)h,the time of symptom resolution in the observation group was significantly lower than that in the control group,and the difference was statistically significant(t=11.268,6.921,8.221,P<0.05);the hemostasis rate of the observation group(95.12%vs 76.19%),treatment efficiency(92.68%vs 73.81%)was higher than that of the control group,the difference was statistically significant(χ^(2)=7.204,5.347,P=0.017,0.015);the incidence of adverse reactions during the treatment of patients in the observation group and the control group(7.32%vs 9.52%),the difference was statistically significant(χ^(2)=0.635,P=0.289).Conclusion The combination of octreotide and omeprazole in the treatment of upper gastrointestinal bleeding has a good overall clinical effect.It can significantly improve the curative effect and reduce the
