摘　　要：目的分析新生儿肺炎患者(日龄<1天)的住院费用构成及其影响因素。方法通过病案信息系统抽取150名不满1日龄的新生儿肺炎诊疗信息,对性别、分娩方式、分娩胎龄是否存在费用差异采用t检验,住院费用的影响因素采用多元线性回归分析。结果足月儿患者的次均住院费用13699.73元,早产儿为20134.27元,前者比后者低6434.54元;正常出生体重组的次均住院费用最低(12653.19元);次均住院费用的主要影响因素是住院天数、是否行机械通气、是否存在吸气三凹征。结论住院天数是影响新生儿肺炎患者住院费用的主要因素;吸气三凹征是新生儿病情严重的生命体征,机械通气是对病情较重的新生儿实行的治疗手段,二者共同说明病情较重的患儿住院费用较高。Objective:To analyze of the distribution of hospitalization expenses and the influencing factors of neonatal pneumonia.Methods:150 medical records with patients less than a day of birth were selected.T test was used to analyze the differences of gender,delivery mode and delivery week in hospitalization expenses distribution.K-W rank sum test was used to analyze the hospitalization expenses among different birth weight groups.Multiple linear regression analysis was used to analyze the influencing factors.Results:The average hospitalization expense of term infants was 13699.73 yuan,and that of premature infants was 20134.27 yuan.The former was 6434.54 yuan lower than the latter.In terms of birth weight,the average hospitalization expense of normal birth weight group was the lowest(12653.19 yuan).The main influencing factors of average hospitalization expenses were the hospitalization days,mechanical ventilation and inspiratory three concave sign.Conclusion:Controlling the length of stay in hospital is the main factor to influence the hospitalization expenses of neonatal pneumonia.Inspiratory three concave sign is a sign of serious illness of newborn.Mechanical ventilation is a treatment method for serious illness of newborn.Both of them indicate that the hospitalization cost of serious illness of newborn is higher.
