摘  要:目的探索以问题为基础的教学法(PBL)联合多学科诊疗(MDT)模式对住院医师肝胆胰系统性临床思维培养的教学效果。方法以武汉大学中南医院肝胆胰外科轮转的规范化培训住院医师为研究对象,实验组(n=60)采用PBL联合MDT教学法训练教学,对照组(n=60)采用传统的LBL联合MDT教学,出科时以自我评价、专家评价、理论测试对自学能力、理论知识掌握度、临床诊疗思维能力、教学模式的认可度、学员学习态度、沟通表达能力、团队合作能力进行评价。结果实验组在自学能力、理论知识掌握能力、临床诊疗思维能力、教学模式认可度、学习态度、沟通表达能力方面均优于对照组(P<0.05),实验组学员对肝胆胰肿瘤疾病的临床共识、指南的掌握能力优于对照组(P<0.05)。结论PBL教学法联合MDT模式在培养住院医师肝胆胰系统性临床思维的教学效果优于讲授式教学法联合MDT模式,学员的接受度高,对学员建立系统性临床思维有所帮助。ObjectiveTo evaluate the teaching effect of problem-based learning(PBL)com-bined with multi-disciplinary treatment(MDT)model in the systematic clinical thinking training of hepatobiliary pancreatic surgery for residents.Method120 standardized training residents from the department of hepatobiliary pancreatic surgery of Zhongnan Hospital of Wuhan University were selected as the research object and randomly divided into the experimental group(n=60)and the control group(n=60).The experimental group accepted PBL combined with MDT teaching method,while the control group was trained by LBL combined with MDT teaching method.After the training,self-learning ability,mastery of theoretical knowledge,clinical diagnosis and treat-ment thinking,recognition of the teaching mode,residents'learning attitude,ability of com-munication and expression as well as team-work ability were assessed by self-evaluation,expert evaluation and theoretical test.ResultsThe experimental group presented better performance in self-learning ability,mastery of theoretical knowledge,clinical diagnosis and treatment thinking,recognition of teaching mode,residents'learning attitude,as well as ability of communication and expression than the control group(P<0.05),while residents'clinical consensus of hepatobiliary pancreatic tumor diseases and ability to master knowledge in the guidebook from the experimental group were also better than those of the residents from the control group(P<0.05).ConclusionPBL teaching method combined with MDT mode performs better teaching effects than LBL teaching method combined with MDT mode in training residents'systematic clinical thinking of hepatobiliary pancreatic surgery.It achieves high acceptance of the residents,and is helpful for students to establish systematic clinical thinking.
