摘　　要：目的分析2013—2018年西部地区社区卫生服务机构卫生资源配置情况及配置的公平性,找出其影响因素,为进一步优化配置卫生资源提供参考依据。方法运用基尼系数、集聚度两项评价指标评价西部地区社区卫生服务机构卫生资源的公平性并分析差异,运用聚类分析找出资源配置不均衡的主要因素。结果“十三五”期间西部地区社区卫生资源配置公平性总体逐年趋好,但是区域间仍存在较大差异;卫生资源按人口分布的公平性高于按地理面积分布的公平性,而经济发展水平是影响资源地理配置分布不均的主要原因。结论在西部地区社区卫生资源配置的问题上,需综合考虑地理、人口、经济及人口流动各类条件,统筹全局,兼顾公平与效率;在人口密度较大土地面积较小的地区,加强卫生资源相对于人口分布的可及性程度,在人口密度较小土地面积较大的地区,加强卫生资源相对于土地面积分布的可及性程度,增强区域内不同地区间资源配置公平性。Objective To analyze the health resource allocation and equity of community health service institutions in western China from 2013 to 2018,and find out the influencing factors,provide the basis for optimizing the allocation of health resources allocation.Methods Gini index and agglomeration degree were used to evaluate the equity of health resources,and the differences were analyzed.Cluster analysis was used to find out the main factors of unbalanced resource allocation.Results During the 13th Five-Year Plan period,the equity of community health resources in western China tended to be better year by year,but there were still great differences among regions.The equity of health resources according to population distribution was higher than that according to geographical area distribution,and economic development was the main reason that affected the uneven distribution of resources in geographical distribution.Conclusion In terms of the allocation of community health resources in western China,geographical,population,economic and population mobility conditions should be considered comprehensively,and fairness and efficiency should be taken into account.In the regions with large population density and small land area,the accessibility of health resources to the population distribution should be enhanced;in the regions with small population density and large land area,the accessibility of health resources to the land area distribution should be enhanced to enhance the fairness of resource allocation among different regions within the region.
