摘 要:目的分析双气相CT扫描下肺内血管体积(IPVV)与慢性阻塞性肺疾病(COPD)患者肺功能指标的相关性。方法选取2019年1月—2020年1月于赣南医学院第一附属医院住院的男性非吸烟COPD患者30例,设置为观察组,同时选取健康的非吸烟男性30例,设置为对照组。利用双气相CT扫描方式测定IPVV,同时予以肺功能测定,比较2组IPVV、肺功能指标的差异,并探究双气相下IPVV与FEV1%及FEV1/FVC的相关性。结果观察组全肺及左右肺IPVV均显著高于对照组(P<0.05);观察组患者FEV1%与FEV1/FVC均显著高于对照组(P<0.05);吸气相IPVV与FEV1%无明显相关性(P>0.05),呼气相IPVV与FEV1%在各部位均呈负相关(r=-0.295~-0.302,P<0.05);吸气相IPVV与FEV1/FVC在各部位均呈负相关(r=-0.316~-0.398,P<0.05),呼气相IPVV与FEV1/FVC各部位均呈负相关(r=-0.341~-0.395,P<0.05)。结论利用双气相CT测定IPVV用于评估COPD患者气道阻塞情况具有可行性,且呼气相CT测定值的整体价值更高。Objective To analyze the correlation between intrapulmonary vascular volume(IPVV)and pulmonary function indexes in patients with chronic obstructive pulmonary disease(COPD)under biphasic CT scan.Methods Thirty male non-smoking patients with COPD who were admitted to the hospital from January 2019 to January 2020 were enrolled as observation group,while another 30 healthy non-smoking male patients were enrolled as control group.IPVV indexes were measured by biphasic CT scan,and pulmonary function was detected.The differences in IPVV and pulmonary function indexes were compared between the two groups.The correlation between IPVV and FEV1%,FEV1/FVC under biphasic CT scan was explored.Results IPVV of whole lung,left and right lungs in observation group were significantly larger than that in control group(P<0.05),FEV1%and FEV1/FVC were significantly higher than those in control group(P<0.05).There was no significant correlation between IPVV of inspiratory phase and FEV1%(P>0.05),IPVV of expiratory phase was negatively correlated with FEV1%(r=-0.295 to-0.302,P<0.05).IPVV of inspiratory phase was negatively correlated with FEV1/FVC(r=-0.316 to-0.398,P<0.05),IPVV of expiratory phase was negatively correlated with FEV1/FVC(r=-0.341 to-0.395,P<0.05).Conclusion It is feasible to evaluate airway obstruction in COPD patients by IPVV measured by biphasic CT.IPVV measured by expiratory phase CT is of higher overall evaluation value.
