摘 要:目的通过联合检测血清25-羟基维生素D_(3)[25-(OH)-D_(3)]、γ-GT、β2-MG水平,探讨其与新生儿高胆症患儿肾脏功能损伤及疾病严重程度的关系,以指导临床诊断及治疗。方法选择2015年1月至2018年6月住院治疗的高胆红素血症新生儿158例作为研究对象,依据患儿的总胆红素监测指标分为轻度、中度、重度3组,另外将同期出生的50例健康新生儿作为对照组。检测3组血清胆红素水平、尿素氮(BUN)、肌酐(Scr)、血清β2-MG、25-(OH)-D_(3)、γ-GT,进行Pearson相关性分析和ROC曲线评价诊断价值。结果SCr、BUN可随着病情加重含量升高,轻度组和中度组、中度组与重度组比较差异无统计学意义(P>0.05);Ccr随着病情加重清除率下降,两两比较差异有统计学意义(P<0.05);γ-GT、β2-MG与TBiL、BUN、Scr呈正相关,与Ccr呈负相关;25-(OH)-D_(3)与TBiL、BUN、Scr呈负相关,与Ccr呈正相关。结论高胆红素血症患儿血清25-(OH)-D_(3)、γ-GT、β2-MG水平均与肾脏功能相关,且与病情的严重程度具有一定联系,三种指标的联合检测对于指导临床治疗具有重要价值。Objective To investigate the correlation between the serum levels of 25-(OH)-D_(3),gamma-GT,beta 2-MG and renal function injury as well as the severity of neonatal hyperbilirubinemia,so as to guide clinical diagnosis and treatment.Methods A total of 158 neonates with hyperbilirubinemia who were treated in our hospital from January 2015 to June 2018 were enrolled in the study.According to the total bilirubin monitoring indexes,these patients were divided into mild group,moderate group and severe group,with 50 cases in each group,moreover,the other 50 healthy subjects were enrolled as control group.The serum levels of bilirubin,BUN,Scr,serum beta 2-mg,25-(OH)-D_(3),and gamma-GT were detected,and the correlation between these indexes were analyzed by using Pearson correlation analysis and ROC curve to evaluate the diagnostic value.Results The levels of SCr and BUN were increased with the aggravation of the disease condition,but,there were no significant differences between mild group and moderate group,and between moderate group and severe group.The clearance rate of Ccr was decreased with the aggravation of the disease condition,there was significant difference between any two groups each other(P<0.05).Moreover thelevels ofγ-GT,β2-MG were positively correlated with those of TBiL,BUN and Scr,however,which were negatively correlated with those of Ccr.25-(OH)-D_(3),in addition,the levels of 25-(OH)-D_(3)were negatively correlated with those of TBiL,BUN and Scr,but,which were positively correlated with those of Ccr.Conclusion The serum levels of 25-(OH)-D_(3),gamma-GT,andβ2-MG in children patients with hyperbilirubinemia are related to the renal function and the severity of disease condition.The combination detection of the three indexes has important value in guiding clinical treatment.
