白光胃镜及超声内镜对食管隆起性病变的诊断价值分析

摘　　要：目的:探讨白光胃镜及超声内镜对食管隆起性病变的诊断价值。方法:回顾性分析我院消化内镜中心2017年1月~2020年1月进行白光胃镜和超声胃镜诊疗的食管隆起性病变患者948例,所有患者均经术后病理结果证实,分析白光胃镜和超声内镜对食管隆起性病变的检出情况。结果:948例食管隆起性病变中,食管息肉87例、食管平滑肌瘤428例、食管囊肿296例、食管脂肪瘤32例、食管外压迫105例。白光胃镜和超声内镜对食管平滑肌瘤的诊断准确率分别为63.32%和87.62%,对食管囊肿的诊断准确率分别为52.03%和72.30%,对食管脂肪瘤的诊断准确率分别为50.00%和90.63%,两组间差异均具有统计学意义(均P<0.05);对食管息肉的诊断准确率分别为97.70%和98.85%,对食管外压迫病变的诊断准确率分别为91.43%和97.14%,两组间均无明显差异(均P<0.05)。结论:白光胃镜和超声内镜对食管息肉和食管外压迫均具有较高的诊断准确率,但超声内镜对于食管平滑肌瘤、囊肿、脂肪瘤的诊断准确率优于白光胃镜。Objective:To investigate the value of white light endoscopy(WLE)and endoscopic ultrasound(EUS)in diagnosis of protrusion lesions of esophagus.Methods:Retrospective analysis was performed on 948 cases of esophageal eminence lesions that were treated by WLE and EUS and confirmed by postoperative pathological detection,in the digestive endoscopy center of our hospital from January 2017 to January 2020.The detection results of esophageal eminence lesions by WLE and EUS were analyzed.Results:There were 87 cases of esophageal polyp,428 cases of leiomyoma,296 cases of cyst,32 cases of lipomyoma and 105 cases of extraesophageal compression.The diagnostic accuracy rate of WLE and EUS for leiomyoma were 63.32%and 87.62%,for cyst were 52.03%and 72.30%,for lipomyoma were 50.00%and 90.63%,respectively(all P<0.05).There were no significant differences in the diagnostic accuracy rate of WLE and EUS for esophageal polyp(97.70%vs 98.85%)and extraesophageal compression(91.43%vs 97.14%)(both P>0.05).Conclusion:Both WLE and EUS have optimal diagnostic accuracy of esophageal polyp and extraesophageal compression.However,EUS could get higher diagnostic accuracy in esophageal leiomyoma,cyst and lipomyoma than that by WLE.
