摘 要:目的:探讨宜昌地区草酸钙肾结石的临床危险因素。方法:回顾分析2018年1月~2019年12月宜昌地区258例草酸钙肾结石住院患者和同期502例非结石健康人群临床资料。采用单因素和多因素logistic回归分析草酸钙肾结石的独立危险因素。结果:单因素logistic回归分析显示,肥胖(OR=3.872,95%CI:2.008~7.121,P<0.001)、高甘油三脂(OR=1.625,95%CI:1.060~2.493,P=0.026)、糖尿病(OR=2.633,95%CI:1.410~4.917,P=0.002)、高尿酸血症(OR=2.892,95%CI:1.744~4.795,P<0.001)与草酸钙肾结石显著相关。多因素logistic回归分析显示,肥胖(OR=2.775,P=0.003)、糖尿病(OR=2.005,P=0.042)和高尿酸血症(OR=2.639,P<0.001)是草酸钙肾结石的独立危险因素。结论:肥胖、糖尿病和高尿酸血症与本地区草酸钙肾结石患病相关,对其早期干预可能降低本地区草酸钙肾结石的发病率。Objective:To explore the clinical risk factors of calcium oxalate kidney stones in Yichang area.Methods:The clinical data of 258 patients with calcium oxalate kidney stones and 502 cases of healthy persons with non-urolithiasis from January 2018 to December 2019 were retrospectively analyzed.The univariate and multivariate logistic regression analysis were used to identify the independent risk factors for calcium oxalate kidney stones.Results:Univariate logistic regression analysis showed that obesity(OR=3.872,95%CI:2.008 to 7.121,P<0.001),high triglyceride(OR=1.625,95%CI:1.060 to 2.493,P=0.026),diabetes(OR=2.633,95%CI:1.410 to 4.917,P=0.002),hyperuricemia(OR=2.892,95%CI:1.744 to 4.795,P<0.001)were significantly associated with calcium oxalate kidney stones.The results of multivariate logistic regression analysis indicated that obesity(OR=2.775,P=0.003),diabetes(OR=2.005,P=0.042),and hyperuricemia(OR=2.639,P<0.001)were independent risk factors for calcium oxalate kidney stones.Conclusion:Obesity,diabetes and hyperuricemia may be associated with the incidence of calcium oxalate kidney stones in Yichang area,which are given early intervention could reduce the prevalence of calcium oxalate kidney stones.
