内踝支撑钢板内固定治疗Lauge-Hansen旋后-内收型Ⅱ度踝关节骨折的效果分析

摘　　要：目的分析内踝支撑钢板内固定治疗Lauge-Hansen旋后-内收型Ⅱ度踝关节骨折的临床效果。方法回顾性分析济源市人民医院2016年1月至2018年12月收治的81例Lauge-Hansen旋后-内收型Ⅱ度踝关节骨折患者的临床资料。根据手术方式将纳入患者分为内踝支撑钢板内固定组43例和螺钉固定组38例。比较两组患者的手术成功率、临床疗效、术后并发症发生情况、骨折愈合时间、恢复完全负重时间、视觉模拟评分法(VAS)评分、美国矫形外科足踝协会(AOFAS)评分。结果术后随访18个月,复查踝关节,结果显示,内踝支撑钢板内固定组手术成功率(97.67%,42/43)高于螺钉固定组(86.84%,33/38),P<0.05;内踝支撑钢板内固定组优良率为95.35%(41/43),高于螺钉固定组的73.68%(28/38),P<0.05。内踝支撑钢板内固定组术后并发症发生率略低于螺钉固定组,但两组比较差异未见统计学意义(P>0.05)。两组骨折愈合时间比较差异未见统计学意义(P>0.05);内踝支撑钢板内固定组恢复完全负重时间[(10.87±1.26)周]少于螺钉固定组[(12.31±1.32)周],P<0.05。两组术前VAS评分、AOFAS评分比较差异未见统计学意义(P>0.05);术后18个月,两组VAS评分均降低,AOFAS评分均升高(P均<0.05),且内踝支撑钢板内固定组VAS评分低于螺钉固定组,AOFAS评分高于螺钉固定组(P<0.05)。结论内踝支撑钢板治疗Lauge-Hansen旋后-内收Ⅱ度踝关节骨折效果好,成功率较高,恢复完全负重时间较短,是安全、有效的手术方式。Objective To analyze the curative effects of internal malleolus support plate internal fixation on Lauge-Hansen supination-adduction typeⅡankle fracture.Methods The clinical data of 81 patients with Lauge-Hansen supination-adduction typeⅡankle fractures admitted to Jiyuan People’s Hospital from January 2016 to December 2018 were analyzed retrospectively.According to the surgical methods,they were divided into medial malleolus support plate internal fixation group(n=43)and screw fixation group(n=38).The surgical success rate,clinical efficacy,postoperative complications,fracture healing time,recovery time to complete weight bearing,visual analogue scale(VAS)score,and American Orthopaedics Foot and Ankle Society(AOFAS)score of the two groups were compared.Results After 18 months of follow-up,the results of ankle joint reexamination showed that the success rate of medial malleolus support plate internal fixation group(97.67%,42/43)was higher than that of screw fixation group(86.84%,33/38),P<0.05.The excellent and good rate was 95.35%(41/43)in medial malleolus support plate internal fixation group,which was higher than the 73.68%(28/38)in screw fixation group,P<0.05.The incidence of postoperative complications in medial malleolus support plate internal fixation group was slightly lower than that in screw fixation group,but there was no significant difference in the incidence between the two groups(P>0.05).There was no statistically significant difference in fracture healing time between the two group(P>0.05).The recovery time of complete weight-bearing in medial malleolus support plate internal fixation group was(10.87±1.26)weeks,which was less than(12.31±1.32)weeks in screw fixation group(P<0.05).There was no significant difference in the VAS score or AOFAS score between the two group before operation(P>0.05).Compared with preoperative scores,the VAS score was significantly reduced,and the AOFAS score was significantly increased 18 months after operation in the two group(P<0.05).VAS score of medial malleolus
