自由体位分娩模式辅助保产无忧方对初产妇产后盆底肌功能及免疫功能的影响

摘　　要：目的探讨自由体位分娩模式辅助保产无忧方对初产妇产后盆底肌功能及免疫功能的影响。方法选取2019年1月—12月我院收治的400例初产妇,按随机数字表法将其分为对照组和观察组,各200例。2组均给予保产无忧方进行治疗,对照组治疗期间给予常规护理进行干预,观察组在对照组的基础上给予自由体位分娩模式进行干预,直至分娩结束,产后随访6周。比较2组产前、产后6周的盆底肌功能情况;产前、产后2 h的血清免疫功能相关指标及炎性因子水平;统计2组随访期间的不良分娩结局发生率。结果与产前比较,产后6周,2组VRP、PFM力量、PFM耐力均升高,且观察组高于对照组(P<0.05);与产前比较,产后2 h,2组血清CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均升高,且观察组高于对照组(P<0.05);产后2 h,2组血清CRP、IL-6、TNF-α水平均低于产前,且观察组低于对照组(P<0.05);随访期间,观察组不良分娩结局发生率低于对照组(P<0.05)。结论自由体位分娩模式辅助保产无忧方,可明显改善初产妇产后盆底肌功能,增强其免疫功能,且具有较高安全性。Objective To investigate the effect of free position delivery mode assisting Baochan Wuyou Fang on postpartum pelvic floor muscle function and immune function of primiparas.Methods Between January 2019 and December 2019,400 primiparas admitted to our hospital for gynecology were selected and divided into control group and observation group by random number table method,with 200 cases in each group.Both groups were given Baochan Wuyou Fang.In addition,the control group was given routine nursing intervention,and the observation group was intervened with free posture delivery mode on the basis of the control group until the end of delivery.Patients in both groups were followed up for 6 weeks after delivery.The pelvic floor muscle function before and 6 weeks after delivery,serum immune function related indexes and inflammatory factor levels before and 2 h after delivery were compared between the two groups,and the incidence of adverse delivery outcomes during the follow-up period of the two groups was counted.Results 6 weeks after delivery,the VRP、PFM strength and PFM endurance of the two groups were all significantly increased,and the observation group was much significantly higher than the control group(P<0.05).2 hours after delivery,the levels of serum CD3^(+),CD4^(+),CD4^(+)/CD8^(+)of the two groups were all significantly increased,and the observation group increased much more significantly(P<0.05).2 hours after delivery,the levels of serum CRP,IL-6,TNF-αof the two groups were all significantly decreased,and the observation group decreased much more significantly(P<0.05).During the follow-up period,the incidence of adverse delivery outcomes in the observation group was lower than that in the control group(P<0.05).Conclusion The free position delivery mode can assist Baochan Wuyou Fang,significantly improving the postpartum pelvic floor muscle function of primiparas,enhancing their immune function,and guaranteeing higher safety.
