摘 要:目的分析ICU患者经膀胱内途径间接监测腹腔内压力(IAP)的影响因素。方法选取2018年1月至2019年12月我院ICU收治的100例腹腔间隙综合征(ACS)患者,经膀胱内途径在平卧位、不同角度半卧位、端坐位、左右侧卧位、中凹位下测量IAP,并经多元线性回归分析IAP测量相关影响因素。结果患者IAP测量值受性别、体位、 BMI及机械通气流量的影响(P <0.05)。在以平卧位为基础体位的情况下,经多元线性回归分析显示,IAP的主要影响因素为性别、 BMI、机械通气流量(P <0.05)。结论在予以ICU腹内高压患者经膀胱内途径IAP测量时,应注意体位、性别、机械通气流量及BMI对IAP测量的影响,确保测定条件适宜,以提高IAP测量准确性。Objective To analyze the influencing factors of indirect monitoring of intra-abdominal pressure(IAP)in ICU patients through intravesical approach.Methods 100 patients with abdominal compartment syndrome(ACS)admitted to our ICU from January 2018 to December 2019 were selected.IAP was measured in supine position,semi recumbent position with different angles,sitting position,left and right lateral position and middle concave position through intravesical approach.The influencing factors of IAP measurement were analyzed by multiple linear regression.Results The IAP measurement value was affected by gender,body position,BMI and mechanical ventilation flow(P<0.05).In the case of supine position as the basic position,the multiple linear regression analysis showed that the main influencing factors of IAP were gender,BMI and mechanical ventilation flow(P<0.05).Conclusions The influence of body position,gender,mechanical ventilation flow and BMI on IAP measurement should be paid attention to in ICU patients with intra-abdominal hypertension through intravesical approach,so as to ensure the suitable measurement conditions and improve IAP measurement accuracy.
