早期嗓音训练对改善甲状腺术后声带麻痹患者嗓音质量的疗效观察

摘　　要：目的探讨早期发声训练对改善甲状腺术后声带麻痹患者嗓音质量的效果。方法选取西安交通大学第一附属医院耳鼻咽喉头颈外科甲状腺术后声带麻痹的48例患者为研究对象,随机分为实验组及对照组(各24例)。实验组自术后1周开始进行系统嗓音训练,对照组不进行干预。对两组术后1周、12周及24周的嗓音主客观评估及频闪喉镜评估数据进行对比分析。结果术后12周,实验组嗓音障碍指数、听感知评估评分中气息声、无力感评分、基频微扰、振幅微扰、最长发声时间及声门闭合程度均与对照组有显著性差异(P均<0.05)。术后24周实验组嗓音障碍指数、最长发声时间仍优于对照组(P均<0.05),其余所有指标两组比较均无显著性差异(P均>0.05)。结论早期嗓音训练可改善声带麻痹患者的嗓音质量。OBJECTIVE To investigate the effect of early voice training on the improving of the voice quality of patients with vocal cord paralysis after thyroid surgery.METHODS Forty-eight patients with vocal cord paralysis after thyroid surgery in our hospital were selected as the research objects and were divided into experimental group and control group according to the principle of randomized control(24 cases each).In the experimental group,voice training was started 1 week after surgery,the control group had nonintervention.Subjective and objective assessment of voice quality and stroboscopic laryngoscopy were performed respectively at 1,12 and 24 weeks after surgery,and the data of the two groups were compared and analyzed.RESULTS At 12 weeks after surgery,the mean voice handicap index(VHI),GRBAS scores of B and A,Jitter,Shimmer,MPT and the degree of glottis closure in the experimental group were significantly different from the control group(P<0.05).At 24 weeks after surgery,VHI and MPT of the experimental group were still better than the control group(P<0.05),and all other parameters showed no significant difference between the two groups(P>0.05).CONCLUSION Early voice training for patients with vocal cord paralysis after thyroid surgery can improve the voice quality of patients in the early stage.
