芪苓健脾配方颗粒辅助治疗肾病综合征脾肾亏虚证30例临床观察

摘　　要：目的观察芪苓健脾配方颗粒辅助治疗肾病综合征脾肾亏虚证的临床疗效。方法将60例肾病综合征脾肾亏虚证患者采用随机数字表法分为2组,各30例。对照组给予西医常规治疗,治疗组在对照组治疗方法的基础上联合芪苓健脾配方颗粒治疗,2组均以4周为1个疗程,治疗2个疗程后比较2组的临床疗效、复发率、不良反应发生情况,以及治疗前后中医证候总积分、实验室检查指标[24 h尿蛋白定量(24 h UPQ)、血浆白蛋白(ALB)、总胆固醇(TC)、三酰甘油(TAG)、血肌酐(Scr)]。结果治疗组总有效率为93.33%(28/30),对照组为70.00%(24/30),2组比较差异有统计学意义(P<0.05);治疗组复发率为3.33%(1/30),对照组为20.00%(6/30),2组比较差异有统计学意义(P<0.05)。2组治疗后中医证候总积分、24 h UPQ、TC、TAG均明显降低,ALB明显升高,与同组治疗前比较差异有统计学意义(P<0.01),且治疗组各项指标改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.05或P<0.01)。2组治疗过程中心电图,血、粪常规,肝功能均未见明显异常。结论芪苓健脾配方颗粒可有效改善肾病综合征脾肾亏虚证患者的临床症状,降低复发率,且安全性高,值得临床推广应用。Objective To observe the clinical efficacy Qiling Jianpi Peifang Keli(芪苓健脾配方颗粒)in adjuvant treatment of nephrotic syndrome with deficiency of spleen and kidney symptom.Methods Sixty cases of nephrotic syndrome with deficiency of spleen and kidney symptom were divided into 2 groups according to random number table method,with 30 cases in each group.The control group was given routine Western medicine treatment,on this basis,the treatment group was combined with Qiling Jianpi Peifang Keli,4 weeks constituted 1 course of treatment for both groups.After 2 courses of treatment,we compared the clinical efficacy,recurrence rate,incidence of adverse reactions,along with TCM symptom total score,laboratory examination indexes[24 hours urine protein quantity(24 h UPQ),plasma albumin(ALB),total cholesterol(TCL),triacylglycerol(TAG)and serum creatinine(Scr)]before and after treatment in the 2 groups.Results The total effective rate was 93.33%(28/30)in the treatment group and 70.00%(24/30)in the control group,and the difference was statistically significant between the 2 groups(P<0.05);the recurrence rate was 3.33%(1/30)in the treatment group and 20.00%(6/30)in the control group,and the difference was statistically significant between the 2 groups(P<0.05).After treatment,TCM symptom total score,24 h UPQ,TC,TAG of the two groups significantly decreased,ALB significantly increased,a nd the difference was statistically significant compared with those before treatment in the same group(P<0.01),and the improvement of various indexes in the treatment group was more significant,and the difference was statistically significant compared with that in the control group after treatment(P<0.05 or P<0.01).There was no significant abnormality in ECG,blood and fecal routine examination and liver function in the 2 groups during the treatment.Conclusion Qiling Jianpi Peifang Keli can effectively improve the clinical symptoms of nephrotic syndrome with spleen and kidney deficiency symptom,reduce the recurrence rate and has high s
