针刺、艾盒灸联合筋伤膏治疗腰背肌筋膜炎32例临床观察

摘　　要：目的观察针刺、艾盒灸联合筋伤膏治疗腰背肌筋膜炎的临床疗效。方法将64例腰背肌筋膜炎患者采用随机数字表法分为治疗组和对照组,各32例。治疗组予针刺、艾盒灸联合筋伤膏治疗,对照组予醋氯芬酸分散片口服治疗。2组均以2周为1个疗程,1个疗程后比较2组的总有效率(近期疗效)以及治疗前后的视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分、身体损害指数(PII)评分;治疗后随访6个月,比较2组的复发率(远期疗效)。结果治疗2周后,对照组总有效率为78.13%(25/32),治疗组为90.63%(29/32),2组比较差异有统计学意义(P<0.05);随访6个月后,对照组复发率为31.25%(10/32),治疗组为9.38%(3/32),2组比较差异有统计学意义(P<0.05)。治疗后2组VAS、ODI、PII评分均明显降低,与同组治疗前比较差异有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。结论针刺、艾盒灸联合筋伤膏治疗腰背肌筋膜炎疗效确切,可明显缓解患者疼痛,恢复腰背肌功能,提高患者生活质量,且具有较好的远期疗效,值得临床推广应用。Objective To observe the clinical efficacy of acupuncture,moxibustion with moxa box combined with Jinshang Gao(筋伤膏)in the treatment of myofascitis of back.Methods Sixty four cases of myofascitis of back were divided into treatment group and control group according to random number table method,with 32 cases in each.The treatment group was given acupuncture,moxibustion with moxa box combined with Jinshang Gao,while the control group was given oral administration of aceclofenac dispersible tablets.Two weeks constituted 1 course of treatment for both groups.After 1 course of treatment,the total effective rate(short-term efficacy),visual analogue scale(VAS)score,Oswestry disability index(ODI)score and physical impairment index(PII)score before and after treatment were compared between the two groups.A 6-month follow-up was conducted after treatment to compare the recurrence rate(long-term efficacy)between the 2 groups.Results After 2 weeks of treatment,the total effective rate was 78.13%(25/32)in the control group and 90.63%(29/32)in the treatment group,and the difference was statistically significant between the 2 groups(P<0.05);after 6 months of follow-up,the recurrence rate was 31.25%(10/32)in the control group and 9.38%(3/32)in the treatment group,and the difference was statistically significant between the 2 groups(P<0.05).After treatment,the VAS,ODI and PII scores of the 2 groups significantly decreased,and the differences were statistically significant compared with those before treatment in the same group(P<0.05),moreover,the decrease in the treatment group was more significant,and the differences were statistically significant compared with those in the control group after treatment(P<0.05).Conclusion It has definite curative effects to use acupuncture,moxibustion with moxa box combined with Jinshang Gao in the treatment of myofascitis of back,which can markedly relieve the pain of patients,restore the function of lumbodorsal muscle,improve the quality of life of patients,and has good long-term curativ
