推拿联合中药熏蒸治疗椎动脉型颈椎病的临床疗效

摘　　要：目的:探究推拿联合中药熏蒸治疗椎动脉型颈椎病的临床疗效。方法:选取2019年5月~2020年5月中山市中医院康复科门诊进行治疗的60例椎动脉型颈椎病患者,按随机数字表法分为对照组与观察组各30例。对照组采用推拿治疗,观察组于推拿后行中药熏蒸治疗。对比两组临床症状及功能状态评分、旋颈试验评分以及血流动力学各指标水平,包括椎动脉平均血流速度(Vm)、阻力指数(RI)、血管搏动指数(PI)。结果:治疗后,两组患者的各症状及功能状态、旋颈试验评分均较治疗前改善(P<0.05),观察组治疗后各临床症状及功能状态评分高于对照组(P<0.05),旋颈试验评分低于对照组(P<0.05)。治疗后,两组患者的Vm、RI及PI均较治疗前改善(P<0.05),且观察组Vm水平高于对照组,RI及PI值低于对照组(P<0.05)。结论:推拿法联合中药熏蒸在椎动脉型颈椎病治疗中获得较好效果,可减轻眩晕、颈肩痛、头痛,改善患者功能状态,调节血流动力学,利于促进预后恢复,值得推广应用。Objective:To explore the clinical efficacy and value of massage combined with traditional Chinese medicine fumigation in the treatment of cervical spondylosis of vertebral artery type.Methods:Sixty patients with cervical spondylosis of vertebral artery type treated in the Rehabilitation Department of Zhongshan Hospital of traditional Chinese Medicine from May 2019 to May 2020 were randomly divided into massage group(n=30)and comprehensive group(n=30).The massage group was treated with massage,and the comprehensive group was treated with traditional Chinese medicine fumigation after massage.The scores of clinical symptoms and functional status,neck rotation test and hemodynamics were compared between the two groups [including vertebral artery mean blood flow velocity(Vm),resistance index(RI),vascular pulsation index(PI)].Results:After treatment,the symptoms and functional status of patients in both groups were improved compared with that before treatment(P<0.05).The clinical symptoms and functional status of patients in the observation group was higher than that of the control group(P<0.05),and the rotator test score was lower than that of the control group(P<0.05).After treatment,Vm,RI and PI of the two groups were improved compared with those before treatment(P<0.05),Vm level of the observation group was higher than that of the control group,RI and PI values were lower than that of the control group(P<0.05).Conclusion:Massage combined with traditional Chine se medicine fumigation has a good effect in the treatment of cervical spondylo sis of vertebral artery type,which can relieve vertigo,neck and shoulder pain and headache,improve the functional state of patients,regulate hemodynamics,and promote the recovery of progno sis,which is worth popularizing and applying.
