摘　　要：目的:观察阿仑膦酸钠辅助Intertan内固定对股骨粗隆间骨折患者血清I型前胶原氨基端前肽(PINP)与I型胶原交联羧基末端肽(CTX)表达的影响。方法:120例股骨粗隆间骨折患者随机分为观察组和对照组各60例。对照组患者采用Intertan内固定治疗,观察组患者在对照组基础上加用阿仑膦酸钠片。治疗并随访1年后,观察两组疗效、并发症、骨痂出现时间及骨折愈合时间,比较两组患者治疗前后关节功能评分、血清PINP、CTX水平变化及治疗后血清炎性因子水平。结果:观察组总有效率为91.7%,并发症发生率为6.67%,均显著优于对照组(P<0.05),且观察组的骨痂出现时间和骨折愈合时间均短于对照组(P<0.05)。治疗后,观察组血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、单核细胞趋化因子蛋白-1(MCP-1)、降钙素原(PCT)水平明显低于对照组,白细胞介素-1(IL-10)、转化生长因子-β(TGF-β)水平明显高于对照组(P<0.05)。治疗后,两组患者的VAS评分与血清CTX水平均较前明显降低,Harris及WHOQOL-100评分和血清PINF水平则较前明显升高(P<0.05);且观察组上述指标均显著优于对照组(P<0.05)。结论:采用阿仑膦酸钠辅助Intertan内固定治疗能使股骨粗隆间骨折患者血清PINP及CTX分泌恢复正常,促进骨折愈合,改善患者关节功能评分。Objective:To observe the effect of alendronate sodium assisted intertan internal fixation on the expression of serum procallagen type I N-terminal propeptide(PINP)and collagen type I crass-linked C-terminal peptide(CTX)in patients with intertrochanteric fracture.Methods:One hundred twenty cases of intertrochanteric fracture were randomly divided into observation group and control group,60 cases in each group.The control group was treated with intertan internal fixation,and the observation group was treated with alendronate sodium tablets on the basis of the control group.After one year of treatment and follow-up,the curative effect,complications,callus appearance time and fracture healing time of the two groups were observed,and the joint function score,serum PINP,CTX levels and serum inflammatory factor levels of the two groups before and after treatment were compared.Results:The total effective rate and complication rate of the observation group were 91.7%and 6.67%,respectively,which were significantly better than those of the control group(P<0.05),and the callus appearance time and fracture healing time of the observation group were shorter than those of the control group(P<0.05).After treatment,the levels of serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),monocyte chemoattractant protein-1(MCP-1)and procalcitonin(PCT)in the observation group were significantly lower than those in the control group,while the levels of interleukin-1(IL-10)and transforming growth factor-β(TGF-β)in the observation group were significantly higher than those in the control group(P<0.05);After treatment,VAS score and serum CTX level of the two groups were significantly lower than before,while Harris and WHOQOL-100 score and serum pinf level were significantly higher than before(P<0.05);and the above indexes of the observation group were significantly better than those of the control group(P<0.05).Conclusion:Alendronate combined with intertan internal fixation can restore the secretion of serum PINP and CTX in patients w
