阿扎胞苷联合CAG方案治疗老年急性髓系白血病疗效及对患者血清VEGF和bFGF的影响观察

摘　　要：目的:观察阿扎胞苷联合CAG方案治疗老年急性髓系白血病(AML)的疗效,及对患者血清血管内皮生长因子(VEGF)和碱性成纤维细胞生长因子(bFGF)的影响。方法:80例老年AML患者随机分为CAG组及联合组,每组40例。CAG组单纯接受CAG方案化疗,联合组接受阿扎胞苷联合CAG方案治疗。治疗8周后观察两组疗效和药品不良反应,比较两组患者外周血细胞、免疫细胞,以及血清VEGF、bFGF水平变化。结果:联合组总有效率为92.50%,明显高于CAG组的72.50%(P<0.05)。治疗后,两组外周血N、Hb及Plt水平和T淋巴亚群CD3+及CD4+/CD8+均较前明显升高(P<0.05),血清VEGF及bFGF水平则较前明显降低(P<0.05);且联合组上述指标均优于CAG组(P<0.05)。两组药品不良反应发生率差异无统计学意义(P>0.05)。结论:阿扎胞苷联合CAG方案可有效提高老年AML疗效,改善患者外周血细胞及免疫细胞水平,降低血清VEGF和bFGF水平,且安全性好。Objective:To observe the efficacy of azacytidine combined with CAG regimen in the treatment of elderly patients with acute myeloid leukemia(AML)and its influence on serum vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF).Methods:80 elderly AML patients were randomly divided into CAG group(CAG schemes)and combined group(azacytidine combined with CAG schemes),40 cases in each group.The CAG group received CAG regimen alone,while the combined group received azacytidine combined with CAG regimen.After 8 weeks of treatment,the curative effects,adverse drug reactions,peripheral blood cell,immune cell levels as well as serum VEGF and bFGF levels were compared.Results:Total effective rate of combined group was 92.5%,which was higher than that of CAG group,72.5%(P<0.05);After treatment,peripheral blood cell N(neutrophil count),Hb and Plt levels for two groups were significantly higher than before(P<0.05);serum VEGF,bFGF and T-lymphocyte subsets(CD3+and CD4+/CD8+)levels for two groups were significantly lower than before(P<0.05);The index above mentioned in combined group were superior to those of CAG group(P<0.05);There was no statistical significance in the incidence of ADR between the two groups(P>0.05).Conclusion:Azacytidine combined with CAG regimen might effectively improve the efficacy of elderly patients with AML,improve the peripheral blood cell and immune cell levels,reduce serum VEGF and bFGF levels,and it was safe.
