摘　　要：目的:分析BiPAP呼吸机结合加热湿化器后无创通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床有效性及安全性。方法:2018年2月-2020年2月收治慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者80例,随机分为两组,各40例。观察组应用BiPAP呼吸机结合加热湿化器后无创通气治疗;对照组单纯使用BiPAP呼吸机进行无创通气治疗。比较两组临床疗效和安全性。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组上机时间及住院时间均明显短于对照组,差异有统计学意义(P<0.05);观察组治疗后动脉血氧分压、动脉二氧化碳分压、心率及呼吸频率等指标水平均低于对照组,差异有统计学意义(P<0.05)。结论:BiPAP呼吸机结合加热湿化器后无创通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭,具有显著疗效,且可缩短患者住院时间。Objective:To analyze the clinical efficacy and safety of BiPAP ventilator combined with heat humidifier for non-invasive ventilation in the treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure.Methods:From February 2018 to February 2020,80 cases of chronic obstructive pulmonary disease patients with type Ⅱ respiratory failure were enrolled,they were randomly divided into two groups with 40 cases in each group.The observation group applied BiPAP ventilator combined with heat humidifier for non-invasive ventilation.The control group only used BiPAP ventilator for non-invasive ventilation.The clinical efficacy and safety of the two groups were compared.Results:The total effective rate of treatment in observation group was higher than that in control group,the difference was statistically significant(P<0.05).The time of using machine and length of hospital stays in the observation group were significantly shorter than those in the control group,the differences were statistically significant(P<0.05).The indicator levels of arterial partial pressure of oxygen,arterial partial pressure of carbon dioxide,heart rate and respiratory rate in the observation group were lower than those in the control group after treatment,the differences were statistically significant(P<0.05).Conclusion:BiPAP ventilator combined with heat humidifier for non-invasive ventilation in the treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure has a significant effect,which can shorten the length of hospital stays.
