摘　　要：近年来,我国药学服务的现状不断发生变化,逐渐从以药品调剂为主发展到以患者为中心的药学监护阶段。通过药物治疗管理,建立药师和患者之间的联系,使患者最终能够实现自我管理。运用中国药物治疗管理(CMTM)的方法学,药师和患者通过预约,进行门诊访谈及文件记录,并定期随访的一种长期管理的模式。In recent years,the current situation of pharmaceutical care in China is constantly changing,gradually developing from drug dispensing to patient-centered pharmaceutical care.To establish the relationship between pharmacists and patients through medication therapy management,so that patients can finally realize self-management.Using the methodology of Chinese Medication Therapy Management(CMTM),pharmacists and patients make appointments,conduct outpatient interviews and document records,and follow up regularly as a long-term management model.
