摘　　要：目的:探究对肾病患者人工动静脉内瘘应用彩超检查血管状况的价值。方法:本研究共80例研究对象,均为肾病患者,所有患者均采用自体人工动静脉内瘘1~6个月,研究对象均选自2017年1月-2019年12月。采用彩色多普勒超声对所有患者的人工动静脉内瘘血管状况进行检查,记录手术一次成功率、检测的血流畅通情况以及相关并发症。结果:80例患者中,手术一次成功患者为79例,一次成功率为98.75%。在80例患者当中,有67例患者显示瘘口处的血流通畅,有13例患者出现并发症,其中,3例患者在静脉流出道形成血栓,3例患者出现静脉瘤样扩张,2例患者出现假性动脉瘤,2例患者为瘘口处狭窄,1例患者为流入道动脉血栓,1例流入道动脉内中膜增厚,1例患者出现盗血综合征。结论:对肾病患者人工动静脉内瘘应用彩超检查可以为造瘘术前的血管选择提供良好的指导,还可以观察到血管状况,了解并发症的情况,为临床制定应对措施提供依据。Objective:To explore the value of using color Doppler ultrasound to check the vascular status of artificial arteriovenous fistulas in patients with nephropathy.Methods A total of 80 subjects in this study were patients with kidney disease.All patients were treated with autologous artificial arteriovenous fistulas for one month to six months.The subjects were selected from January 2017 to December 2019.Color Doppler frontal ultrasound was used to check the vascular status of all patients with artificial arteriovenous fistulas,and record the first-time success rate of the operation,the detected blood flow,and related complications.Results Among the 80 patients,79 patients had a successful one-time operation,and the one-time success rate was 98.75%.Among the 80 patients,67 patients showed unobstructed blood flow at the fistula,and 13 patients developed complications:3 patients developed thrombosis in the venous outflow tract,3 patients developed venous tumor-like dilatation,and 2 patients developed venous outflow tract thrombosis.Pseudoaneurysm,2 patients had fistula stenosis,1 patient had inflow arterial thrombosis,1 had inflow arterial intima-media thickening,and 1 patient had steal blood syndrome.Conclusion The application of color Doppler ultrasound examination of artificial arteriovenous fistula in patients with nephropathy can provide a good guide for the choice of blood vessels before fistula surgery.It can also observe the state of blood vessels,understand the complications,and provide a basis for clinical countermeasures.
