上颌窦出血坏死性息肉临床特征分析

摘　　要：目的探讨上颌窦出血坏死性息肉的临床特征,减少误诊。方法回顾性分析10例上颌窦出血坏死性息肉患者的临床资料,包括临床表现、CT、MRI及病理学特征等。结果10例患者均在全麻下行鼻内镜鼻窦手术,术后均经病理证实,临床症状明显改善,随访10个月~3年,未见复发。结论上颌窦出血坏死性息肉病变多位于单侧鼻腔上颌窦,以鼻塞、鼻出血为主要临床表现,部分病例还表现为涕中带血、面部肿胀麻木、溢泪等。T2W1肿物中心呈不均匀高信号影、边缘被不规则低信号影包绕或分隔是该病最具特征性的影像学表现,增强后肿物呈结节状、菜花状,强化明显。本病确诊仍需依靠病理,鼻内镜手术是唯一有效的治疗手段,预后良好。Objective To summarize the clinical features of maxillary sinus hemorrhagic necrotic polyps and reduce the misdiagnosis.Methods The clinical data of 10 cases of maxillary sinus hemorrhagic necrotic polyps were retrospectively analyzed,including clinical manifestations,CT,MRI and pathological manifestations.Results All the 10 patients underwent endoscopic sinus surgery under general anesthesia.Postoperative clinical symptoms were significantly improved.After follow-up ranging from 10 months to 3 years,no tumor recurrence was observed.Conclusion The lesions of maxillary sinus hemorrhagic necrotic polyps are mostly located in unilateral nasal and maxillary sinuses.The main clinical manifestations are nasal congestion and epistaxis,and some cases also show blood-stained nasal discharge,facial swelling and numbness and tears spill.T2W1 tumor showed uneven high signal shadow in the center,and irregular low signal shadow around the edge.After enhancement,the tumor showed nodular,patchy and cauliflower shape enhancement.The diagnosis still needs to rely on pathological examination.Endoscopic sinuses surgery is the only effective treatment with good prognosis.
