腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠患者的效果

摘　　要：目的:探讨腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠效果。方法:选取本院2018年1月-2019年1月101例输卵管妊娠患者,按手术方式分为对照组(腹腔镜下保留输卵管妊娠病灶清除手术,n=50)和观察组(腹腔镜下输卵管抽芯切除术+宫角缝扎术,n=51)。比较两组手术相关指标、性激素水平及异位妊娠发生率。结果:两组手术时间、住院时间比较差异无统计学意义(P>0.05),术中出血量观察组低于对照组(P<0.05)。术后1、6个月,两组血清抗缪勒管激素、雌二醇、卵泡刺激素和黄体生成素水平比较差异无统计学意义(P>0.05),术后观察组异位妊娠发生率低于对照组(P<0.05)。结论:腹腔镜下输卵管抽芯切除术联合宫角缝扎治疗输卵管妊娠患者能有效降低患者术中出血量,降低异位妊娠发生率,对患者手术时间、住院时间及性激素水平的影响与腹腔镜下保留输卵管妊娠病灶清除手术无显著差异。Objective:To explore the effect of laparoscopic core-pulling resection of fallopian tube combined with uterine corneal suture in the treatment of tubal pregnancy.Method:A total of 101 patients with tubal pregnancy in our hospital from January 2018 to January 2019 were selected and divided into the control group group(laparoscopic tubal-preserving pregnancy lesion removal surgery,n=50)and the observation group(laparoscopic core-pulling resection of fallopian tube combined with uterine corneal suture,n=51)according to the surgical method.Compared the surgery-related indexes,sex hormone levels and the incidence of ectopic pregnancy.Result:There were no significant differences in the operation time and hospital stay between the two groups(P>0.05).The intraoperative blood loss of the observation group was lower than that of the control group(P<0.05).There were no significant differences in the levels of anti-Mullerian hormone,estradiol,follicle stimulating hormone and luteinizing hormone 1,6 months after surgery(P>0.05).The incidence of ectopic pregnancy in the observation group was lower than that in the control group(P<0.05).Conclusion:Laparoscopic core-pulling resection of fallopian tube combined with uterine corneal suture in the treatment of tubal pregnancy can effectively reduce the amount of intraoperative blood loss and reduce the incidence of ectopic pregnancy,and there are no significant differences in the effect of surgical duration,length of hospital stay and sex hormone levels compared with laparoscopic tubal-preserving pregnancy lesion removal surgery.
