消风止痒汤联合丙酸倍他米松乳膏治疗神经性皮炎的疗效观察

摘　　要：目的观察消风止痒汤联合丙酸倍他米松乳膏治疗神经性皮炎的疗效。方法选取神经性皮炎患者129例,按随机数字表法分为对照组(n=64)和研究组(n=65),对照组予以丙酸倍他米松乳膏外涂患处及口服氯雷地定片治疗,研究组在对照组基础上给予消风止痒汤口服治疗,比较两组患者临床疗效、中医症候积分、不良反应、血清学指标(5-羟色胺、P物质、组胺、β-内啡肽)。结果治疗21 d后,研究组疗效优于对照组,瘙痒程度、皮损形态、肥厚程度、色素沉着、皮损面积、失眠、心烦、便秘评分及血清5-羟色胺、P物质、组胺、β-内啡肽水平均低于对照组(均P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。研究组复发率低于对照组(P<0.05)。结论消风止痒汤联合丙酸倍他米松乳膏治疗神经性皮炎疗效好,可有效改善患者临床症状,降低引起瘙痒的相关因子水平。Objective To observe the efficacy of Xiaofeng Zhiyang Decoction combined with Betamethasone Dipropionate Cream for the treatment of neurodermatitis.Methods A total of 129 patients with neurodermatitis were enrolled and divided into control group(n=64)and study group(n=65)according to the random number table method,the control group was treated with external application of Betamethasone Dipropionate Cream and oral administration of Loratadine Tablets,and the study group was given Xiaofeng Zhiyang Decoction orally on the basis of the therapy in the control group.Clinical efficacy,traditional Chinese medicine(TCM)symptom score,adverse reactions,serological indexes(5-hydroxytryptamine,substance P,histamine,β-endorphin)were compared between the two groups.Results After 21 days of treatment,the study group obtain superior efficacy than the control group,and exhibited lower scores for symptoms including pruritus severity,skin lesion morphological features,hypertrophic degree,pigmentation,skin lesion extension,insomnia,dysphoria and constipation,and lower levels of serum 5-hydroxytryptamine,substance P,histamine andβ-endorphin as compared with the control group(all P<0.05).There was no statistically significant difference in incidence rate of adverse reactions between the two groups(P>0.05).The study group yielded a lower recurrence rate than the control group(P<0.05).Conclusion Xiaofeng Zhiyang Decoction combined with Betamethasone Dipropionate Cream achieves a favorable efficacy for the treatment of neurodermatitis,which can effectively improve patients′clinical symptoms and reduce the levels of related factors causing pruritus.
