艾灸辅助治疗并发不宁腿综合征的血液透析患者的临床效果

摘　　要：目的观察艾灸辅助治疗并发不宁腿综合征(RLS)的血液透析患者的临床效果。方法将60例并发RLS的血液透析患者随机分为观察组和对照组,各30例。两组患者均给予常规血液透析治疗联合加巴喷丁治疗,观察组患者同时给予艾灸治疗。观察两组患者的RLS疗效,并比较两组治疗前后血红蛋白、血钙、磷、肌酐、甲状旁腺激素(PTH)、白细胞介素1β(IL-1β)水平及国际不宁腿量表(IRLS)评分、匹兹堡睡眠质量指数(PSQI)评分。结果治疗后,观察组总有效率为86.67%,对照组为63.33%,观察组疗效及总有效率均优于对照组(均P<0.05)。治疗后,观察组患者血磷、血清肌酐、PTH和IL-1β水平及IRLS评分、PSQI均低于治疗前和对照组,血红蛋白和血钙水平高于治疗前和对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论艾灸联合加巴喷丁治疗血液透析并发RLS具有较好效果,可有效改善患者血生化指标、临床症状和睡眠质量,且不良反应较少,可在临床治疗中推广应用。Objective To observe the clinical efficacy of adjuvant moxibustion therapy for concomitant restless leg syndrome(RLS)in hemodialysis patients.Methods Sixty hemodialysis patients with concomitant RLS were randomly divided into observation group and control group,with 30 cases in each group.Patients in the two groups were given routine hemodialysis combined with gabapentin therapy,simultaneously,patients in the observation group were given moxibustion therapy.The efficacy for RLS was observed in patients of the two groups,in addition,the levels of hemoglobin,serum calcium,phosphorus,creatinine,parathyroid hormone(PTH)and interleukin 1β(IL-1β),as well as the International Restless Leg Syndrome Rating Scale(IRLS)score and the Pittsburgh Sleep Quality Index(PSQI)score were compared between the two groups before and after treatment.Results After treatment,the total effective rate was 86.67%in the observation group and 63.33%in the control group,the observation group had superior efficacy and total effective rate than the control group(all P<0.05).After treatment,the observation group exhibited lower levels of serum phosphorus,serum creatinine,PTH and IL-1β,decreased IRLS score and PSQI score,and higher hemoglobin and serum calcium levels as compared with the pretreatment and the control group(P<0.05).There was no statistically significant difference in incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Moxibustion combined with gabapentin therapy achieves a favorable efficacy for concomitant RLS in hemodialysis,which can effectively improve patients′biochemical indexes,clinical symptoms and sleep quality,and cause few adverse reactions.It is worthy of promotion and application in clinical treatment.
