侧后入路椎间孔镜行腰椎间孔成形术相关解剖数据测量

摘　　要：目的:测量侧后入路椎间孔镜下行腰4/5、腰5/骶1节段椎间孔成形术的相关数据,为镜下成形术及器械改良提供依据。方法:解剖经福尔马林浸泡的6具成人尸体脊柱标本,测量腰5上关节突前1/4点至腰4出口根的距离,腰5上关节突前1/4点至腰4腰5椎体后缘连线的距离,骶1上关节突前1/4点至腰5出口根的距离以及骶1上关节突前1/4点至腰5骶1椎体后缘连线的距离。结果:腰5上关节突前1/4点至腰4出口根的距离为11.05±2.52 mm,腰5上关节突前1/4点至腰4腰5椎体后缘连线的距离为10.34±3.21 mm,骶1上关节突前1/4点至腰5出口根的距离为13.27±4.22 mm,骶1上关节突前1/4点至腰5骶1椎体后缘连线的距离为11.25±3.13 mm。结论:测量侧后入路椎间孔镜下行腰4/5、腰5/骶1节段椎间孔成形术的相关数据,可为改良镜外环锯的直径大小设计提供依据。侧后入路经皮椎间孔镜关节突成形时,在腰4/5、腰5/骶1节段环锯直径可以达到8 mm、8.5 mm或9 mm,能安全有效行关节突成形。Objective:Measure the anatomical data of the intervertebral foramen of L4/5 and L5/S1,which provides basis for forming technology and equipment improvement under spinal endoscopy.Methods:Adult cadaveric spine specimens were dissected.The distance between the 1/4 point of the superior articular process of L5 and the exit root of the L4,and the distance between the 1/4 point of the superior articular process of L5 and the posterior border of lumbar intervertebral disc of L4/5 were measured.The distance between the 1/4 point of the superior articular process of S1 and the exit root of the L5,and measure the distance between the 1/4 point of the superior articular process of S1 and the posterior border of lumbar intervertebral disc of L5/S1 were measured.Results:The distance between the 1/4 point of the superior articular process of L5 and the exit root of the L4 was 11.05±2.52 mm.The distance between the 1/4 point of the superior articular process of L5 and the posterior border of lumbar intervertebral disc of L4/5 was 10.34±3.21 mm.The distance between the 1/4 point of the superior articular process of S1 and the exit root of the L5 was 13.27±4.22 mm.The distance between the 1/4 point of the superior articular process of S1 and the posterior border of lumbar intervertebral disc of L5/S1 was 11.25±3.13mm.Conclusion:In percutaneous transforaminal endoscopic surgery,in order to be safe and effective for facet forming at the L4/5 or L5/S1 trephine diameter can reach 8mm,8.5mm and 9mm.
