摘　　要：目的:分析维持性血液透析(MHD)患者的抑郁状况及其影响因素。方法:选取2017年11月—2018年6月进行MHD的132例患者,由经过统一培训的调查员进行面对面询问式问卷调查。将抑郁标准分作为因变量,以性别、年龄、工作状况、家庭人均月收入作为自变量,建立多元回归模型,筛选出影响血液透析患者抑郁状况的主要因素。结果:132例长期血液透析患者中71例患者抑郁自评量表(SDS)评分≥50分,占53.8%。132例MHD患者SDS总评分为(51.9±8.9)分,显著高于国内常模。其中影响血液透析患者抑郁状况的两个主要影响因素分别是工作状况、家庭人均月收入。结论:长期血液透析患者抑郁的发生率较高,工作状况、家庭人均月收入是两个重要影响因素。在临床工作中应注意患者的心理状况,从多方面采取措施进行积极干预。Objective:To investigate the depressive status of maintenance hemodialysis(MHD)patients and its influencing factors and assess the depression in dialysis patients,then analyze the influencing factors of depression and give clinical intervention later.Methods:All 132 cases of maintenance hemodialysis patients in the blood purification center of Zhengzhou People′s Hospital from November 2017 to June 2018 were selected as the study subjects.All selected patients were interviewed face-to-face by a uniformly trained investigator.The standard scores of depression were used as dependent variables.Multiple regression models were established based on gender,age,work status and family monthly income as the independent variables,and the main factors influencing the depression of hemodialysis patients were screened out.Results:In 132 patients with long-term hemodialysis,71 patients had sodium dodecyl sulfate(SDS)score greater than or equal to 50,and 53.8%had depression.The total SDS score of 132 MHD patients was(51.9±8.9),which was significantly higher than the domestic norm.The two main factors affecting the depressive status of hemodialysis patients are work status and family income per capita.Conclusion:Long-term hemodialysis patients have a high incidence of depression.Work status and family per capita monthly income are two important factors.The depression scores of widowed or divorced patients are relatively high,and those of short dialysis age(within 5 years)and long dialysis age(over 10 years)are also relatively high.In clinical work,attention should be paid to the psychological status of patients,and positive interventions should be taken from various aspects.
