摘  要:目的探讨扩散张量成像(DTI)序列各向异性分数(FA)值变化对急性视神经炎(ON)的诊断价值。方法回顾性分析11例经临床证实的急性ON患者,均行常规MRI和DTI检查,测量患侧和健侧视神经的FA值,并收集患者的最佳矫正视力、瞳孔直径、视野平均缺损(MD)、视野模式标准差(PSD)、闪光视觉诱发电位(F-VEP)的P波潜伏期、振幅等临床数据对视觉传入系统进行评估,分析不同检查方法与FA值的相关性。结果急性ON患者患侧视神经平均FA值显著低于健侧,差异有统计学意义(P<0.001)。患侧与健侧最佳矫正视力、瞳孔直径、MD、PSD、F-VEP的P波振幅及潜伏期差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,FA值与P波潜伏期呈负相关(r=-0.658,P=0.039),与MD呈负相关(r=-0.723,P=0.012),与P波振幅呈正相关(r=0.652,P=0.041),与最佳矫正视力、瞳孔直径、PSD无相关性。结论FA值可以反映出视神经轴突损伤和髓鞘脱失的病理性改变,对急性ON诊断有重要价值。Ohjective To explore the application value of changes in fractional anisotropy(FA)value of diffusion tensor imaging(DTI)examination in the diagnosis of acute optic neuritis(ON).Methods 11 patients with clinical diagnosis of acute ON were enrolled and examined by conventional MRI and DTI,the FA value of the afcted side and the healthy side were measured.The best corrected visual acuity,pupil diameter,mean defect(MD)of visual field,pattern standard deviation(PSD)of visual field,P wave latency and amplitude of flash visual evoked potential(F-VEP)were collected to evaluate the visual afferent system,and analyzed the correlation between different inspection methods and the FA value.Results The average FA value of afected optic nerves were declined obviously in.comparison to the healthy optic nerves,and there were statistically significant dffrences(P<0.001).There were staitially significant differences in the best corrcted visual acuity,pupil diameter,MD,PSD,P wave latency and amplitude of F-VEP between the afected side and the healthy side(P<0.05).Pearson correlation analysis results showed that the FA value was negatively correlated with P wave latency(r=-0.658,P=0.039)and MD(r=-0.723,P=0.012),and positively correlated with P wave amplitude(r=0.652,P=0.041),but no correlation with the best corrected visual acuity,pupil diameter and PSD.Conclusion FA value can reflect the pathological changes of optic nerve axon injury and demyelination,which is of great value in the diagnosis of acute ON.
