摘  要:目的探讨低剂量螺旋CT在新型冠状病毒肺炎(COVID-19)早期诊断的应用价值。方法选取确诊为COVID-19患者24例,均行胸部低剂量薄层CT扫描,回顾性分析所有病例的临床资料和CT表现。结果24例患者首次及第2次咽拭子核酸检测均为阴性,4例患者第3次为阳性,15例患者第4次为阳性,4例患者第5次为阳性,1例患者第8次为阳性。24例患者病变过程中2例患者病变始终未见明显异常征象,1例病变仅累及单侧肺叶,呈局限性磨玻璃影。21例病变累及双侧肺并见双肺多发结节样、斑片状磨玻璃影、实变影。其中有4例为危重型,2例表现为双肺弥漫性淡薄影并肺纹理增粗。所有病例纵隔与肺门内未见明显淋巴结增大,3例危重型COVID-19患者胸腔内见少量积液。结论在COVID-19早期诊断时,双肺低剂量扫描,使患者在接受低剂量的同时清晰显示肺部病灶的形态、密度、个数及病变的演变过程,其中大部分患者肺部CT征象早于核酸检测结果。Ohjective To investigate the value of low dose spiral CT in the early diagnosis of COVID-19.Methods Twenty-four cases of COVID-19 were collected.All cases underwent low dose thin-section CT scanning of the chest.The clinical data and CT findings of all cases were analyzed retrospectively.Results The first and second pharyngeal wipe nucleic acid tests were negative in 24 patients,he third was positive in 4 patients,the fourth was positive in 15 patients,the fifth was positive in 4 patients,and the eighth was positive in 1 patient.During the course of the disease in 24 patients,2 patients had no obvious abnormal signs in the lesion,and 1 patient had lesion involving only one lobe,showing a localized ground-glass opacity.In 21 cases,the lesions involved the lungs and showed multiple nodular,patchy ground glass opacities and consolidation.Among them,there were 4 cases of severe type,and 2 cases showed diffuse hin shadow and thickened lung markings.No significant lymph node enlargement was observed in the mediastinum and hilum in all cases,and a small amount of effusion shadows were observed in the thoracic cavity of 3 critically ill patients with COVID-19.Conclusion In the early diagnosis of COVID-19,low dose scanning can clearly show the shape,density,number and evolution of pulmonary lesions while receiving low-dose,and the pulmonary CT signs of most patients are earlier than the nucleic acid detection results.
