摘　　要：目的探讨多层螺旋CT(MSCT)平行与垂直肿瘤长径多平面重建(MPR)对结肠癌术前TN分期的诊断价值。方法选取CT全腹增强检查及平行与垂直肿瘤长径MPR拟诊并经手术病理证实为结肠癌的患者200例,盲法术前对肿瘤进行TN分期,并与术后病理分期对比,研究TN分期的准确率、灵敏度、特异度、阳性符合率,评价分期的一致性。结果T4a期CT与病理分期一致性中等;≤T2、T3、T4b期具有高度一致性。N1c期CT与病理分期一致性中等;N1a、N1b、N2a、N2b期具有高度一致性,≤N0一致性极高。结论MSCT平行与垂直肿瘤长径MPR对结肠癌能够较准确术前TN分期。Objective To investigate the value of multi spiral computed tomography(MSCT)parallel and vertical diameter to tumor on multiplanar reconstruction(MPR)in the determination of preoperative TN staging of colon cancer.Methods 200 cases of colon cancer patients were enrolled in the study with abdomen enhancement CT and MPR images,which were confirmed by surgery pathology.The preoperative TN staging of the tumor was determined blindly and compared with postoperative pathological stating.The accuracy,sensitivity,specificity,and positive coincidence rate of TN staging and the consistency of the staging evaluation were analyzed.Results The moderate consistency was found between T4a stage of CT and pathological staging,and high consistency was seen between≤T2,T3 and T4b stage of CT and the pathological staging.N1c stage of CT was moderately consistent with pathological staging,Nla,N1b,N2a and N2b stages of CT were highly consistent with pathological staging,and≤N0 of CT was extremely consistent with pathological staging.Conclusion MSCT parallel and vertical diameter to tumor long on MPR can accurately determine the preoperative TN staging of colon cancer.
