摘　　要：目的评估冠状动脉联合髂动脉CT血管成像(CTA)一站式扫描对髂动脉图像质量的影响。方法行肾移植术前评估的患者273例,其中行CTA一站式扫描者为联合组(162例),行单独髂动脉扫描者为对照组(111例)。测量并比较2组髂动脉的图像质量、辐射剂量和住院时间。结果与对照组相比,联合组右侧髂总动脉主观评分高于对照组(3.93±0.26 vs 3.85±0.36,P=0.042),右侧髂内动脉CT值高于对照组[(338.58±78.29)HU vs(316.96±79.52)HU,P=0.047];所有分段的对比噪声比均高于对照组(P<0.05);有效辐射剂量较对照组减少约30%;住院天数明显少于对照组(8.53±3.35 vs 10.54±3.92,P<0.001)。结论设备条件允许的前提下,推荐肾移植术前评估选择冠状动脉加髂动脉联合扫描,能保证图像质量,减少对比剂剂量和住院时间,减轻患者负担,对肾脏终末期患者有重大临床意义。Objective To evaluate the effect of one-stop coronary artery combined with iliac artery CT angiography(CTA)on the image quality of iliac artery.Methods A total of 273 patients who underwent preoperative evaluation of renal transplantation were analyzed.Patients with one-stop scan(n=162)were assigned to the combined group.Patients with the single iliac artery scan(n=111)were assigned to the control group.The image quality of iliac arteries,radiation dose and hospital stay time of the two groups were measured and compared.Results Compared with the control group,the subjective score of the right common iliac artery was higher than that n the control group(3.93±0.26 vs 3.85±0.36,P=0.042).The CT value of the right internal iliac artery was higher than that in the control group[(338.58±78.29)HU vs(316.96±79.52)HU,P=0.047].The contrast noise ratios of all segments of the iliac artery were higher than that in the control group(P<0.05).The effective radiation dose of the only iliac artery scan was reduced about 30%compared with the control group.The hospital stay time was significantly less than that in the control group(8.53±3.35 vs 10.54±3.92,P<0.001).Conclusion It is recommended to select the combination of coronary artery and iliac artery for preoperative evaluation of renal transplantation if equipment condition is permitted.The com bination scan can ensure the image quality,reduce the dose of contrast agent,the hospital stay time,and the burden of patients,which has great clinical significance for patients with end-stage renal disease.
