摘 要:目的比较分析贵州省脑膜炎球菌多糖疫苗(meningococcal polysaccharide vaccine,MPV)推广使用和纳入免疫规划策略的成本-效果。方法根据贵州省MPV接种、流脑发病情况,采用成本-效果分析法,计算贵州省不同阶段推广使用MPV和将MPV纳入免疫规划策略的成本-效果比值(cost-effectiveness ratio,CER)。结果1999—2015年,贵州省共接种6285.73万剂次MPV,共减少流脑发病94069例,减少致残13170例,减少死亡10710例;推广使用MPV、将MPV纳入免疫规划免疫策略总投入成本8.20亿元,26年间,接种MPV每减少1例流脑病例投入的成本为6952元,其中,1990—2005年推广使用MPV、2006—2015年纳入免疫规划策略每减少1例流脑病例投入的成本分别为4915.64、614.59元。结论接种MPV对降低贵州省流脑发病水平具有良好的成本-效果,2006—2015年将MPV纳入免疫规划策略的成本-效果明显优于1990—2005年推广使用MPV策略。Objective To compare and analyze the cost-effectiveness of promoting the use of meningococcal polysaccharide vaccine(MPV)and incorporating MPV into the immunization planning strategy in Guizhou Province.Methods Cost-effectiveness ratio(CER)of promoting the use of MPV and incorporating MPV into the immunization planning strategy in Guizhou Province at different stages was calculated based on MPV vaccination and rheumatic morbidity in Guizhou Province.Results From 1999 to 2015,a total of 62857300 doses of MPV were administered in Guizhou Province,reducing the incidence of epidemic encephalitis by 94069 cases,disability by 13170 cases,and death by 10710 cases.The total cost of promoting the use of MPV and including MPV in the immunization planning strategy was 820 million yuan,and the total cost of MPV vaccination was 6952 yuan per case of reduction of epidemic encephalitis during 26 years,of which,the cost of promoting the use of MPV from 1990 to 2005 and the cost of incorporating MPV into the immunization planning strategy from2006 to 2015 were 4915.64 yuan and 614.59 yuan per case of reduction of epidemic encephalitis,respectively.Conclusion MPV vaccination has good cost-effectiveness in reducing the incidence of epidemic encephalitis in Guizhou Province,and the cost-effectiveness of including MPV in the immunization planning strategy in 2006-2015 was significantly better than that of promoting MPV in 1990-2005.
