摘　　要：总结了世界载人航天工程历史上导致宇航员牺牲的失利案例,分析了这些失利背后的逻辑,并对当前汲取教训过程中存在的问题,从始终敬畏所从事的事业、正确认识责任和教训、深刻牢记海恩法则和墨菲定律、时刻保持风险意识和问题意识、建设可靠的组织等方面提出了启示。Quality problems that led to the death of astronauts in the history of the world's manned space are analyzed.Root causes of these accidents are scrutinized and lessons learned are drawn.Some thoughts on related to our missions,understanding of responsibilities and lessons learned correctly,keeping in mind Hein's Law and Murphy's Law,maintaining risk awareness and problem awareness and building a reliable organization are then put forward.
