摘  要:目的探讨细胞免疫应答变化与支气管哮喘疾病进展的相关性。方法选取95例支气管哮喘患者为观察组,选取同期体检的健康人95例为对照组,记录研究对象辅助性T细胞1(T helper cells 1,Th1)、Th2、Th17和调节性T细胞(regulatory cell,Treg)比例,记录白细胞介素-2(interleukin-2,IL-2)、IL-4、γ干扰素(interferon-γ,IFN-γ)、IL-10、IL-17、转化生长因子-β(transforming growth factor-β,TGF-β)、IL-22 mRNA和蛋白表达水平,测定研究对象血液中CD3^(+)、CD4^(+)、CD8^(+)水平,并计算CD4^(+)/CD8_(+)。结果观察组Treg和Th1细胞占比低于对照组,Th2和Th17细胞占比高于对照组,差异有统计学意义(P<0.05),观察组Th1/Th2低于对照组,Th17/Treg高于对照组,差异有统计学意义(P<0.05);观察组CD4+/CD8+、CD8+、CD3+和CD4+水平均低于对照组,差异有统计学意义(P<0.05);观察组TGF-β、IL-2和IFN-γmRNA表达水平低于对照组,IL-22、IL-17、IL-4和IL-10 mRNA表达水平高于对照组,差异有统计学意义(P<0.05);观察组TGF-β、IL-2和IFN-γ蛋白表达水平低于对照组,IL-22、IL-17、IL-4和IL-10蛋白表达水平高于对照组,差异有统计学意义(P<0.05)。结论支气管哮喘患者CD4^(+)/CD8^(+)、Th17/Treg和Th1/Th2比值较健康人群发生变化,支气管哮喘疾病的进展可能与这些细胞免疫应答的异常变化密切相关。Objective To investigate the correlation between changes in cellular immune responses and progression of severe bronchial asthma.Methods Ninty-five patients with severe bronchial asthma who were treated in Anyang District Hospital and Henan People′s Hospital from May 2017 to May 2018 were selected as the study group;and 95 healthy people who were examined in Anyang District Hospital and Henan People′s Hospital during the same period were selected as the control group.T helper cells 1(TH1),Th2,Th17 and regulatory cell(Treg)ratio were assessed.Interleukin-2(IL-2),IL-4,interferon-γ(IFN-γ),IL-10,IL-17,transforming growth factor-β(TGF-β),IL-22 mRNA and protein expression levels were also recorded.The blood CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)levels were compared.Results The proportion of Treg and Th1 cells in the study group was significantly lower than that in the control group(P<0.05);and the proportion of Th2 and Th17 cells was significantly higher than that of the control group(P<0.05).The Th1/Th2 in the study group was significantly lower than that in the control group,and the Th17/Treg was significantly higher than the control group(P<0.05).The levels of CD4^(+)/CD8^(+),CD8^(+),CD3^(+)and CD4^(+)in the study group were significantly lower than those in the control group(P<0.05).The expression levels of TGF-β,IL-2 and IFN-γprotein in the study group were significantly lower than those of the control group,and the expression levels of IL-22,IL-17,IL-4 and IL-10 were significantly higher than those of the control group(P<0.05).Conclusions The ratio of CD4^(+)/CD8^(+),Th17/Treg and Th1/Th2 in patients with severe bronchial asthma are different with healthy people.The progression of severe bronchial asthma may be closely correlated to the abnormal changes of these cellular immune responses.
