大面积特重度烧伤“干”转“湿”治疗效果分析

摘　　要：目的总结分析大面积特重度烧伤"干"转"湿"治疗的关键节点及技术要领。方法对2012年1月至2018年12月来自中国、埃及、印尼、菲律宾的5家医疗中心的37例早期行干燥疗法治疗的大面积特重度烧伤患者改用烧伤创疡再生医疗技术治疗,观察治疗效果:结果 37例患者中,除1例伴有严重多发复合伤患者于治疗第25天死于肺部耐药菌感染外,其余36例患者均顺利完成"干"转"湿"治疗,创面于治疗第17-76 d [(39.5±11.2) d]愈合,住院费用为(13.52±6.30)万元。结论早期行干燥疗法治疗的大面积特重度烧伤患者改用烧伤创疡再生医疗技术治疗,能够实现"干"转"湿"的治疗目的,且疗效显著,安全可靠,值得临床借鉴。Objective To summarize and analyze the key nodes and technical key points of switching from the dry therapy to the moist therapy in treating extensive extremely-severe burns.Methods Thirty-seven patients with extensive extremely-severe burns from five medical centers in China,Egypt,Indonesia and Philippine between January 2012 and December 2018 were switched to receive the moist therapy after treatment of the dry therapy at the early stage,and the therapeutic effect was observed.Results Of the 37 patients,36 patients successfully completed the treatment switch from the dry therapy to the moist therapy,except one patient complicated with severe multiple combined injuries who died of pulmonary infection with drug-resistant bacteria on day 25 of treatment.The wounds of the 36 patients were healed on day 17-76[(39.5±11.2)d]of treatment,and the hospitalization cost was(13.52±6.30)RMB.Conclusion For extensive extremely-severe burns,switching the early treatment with the dry therapy to the regenerative medical technology for burns,wounds and ulcers-the moist therapy can realize the treatment goal of replacing the dry therapy with the moist therapy,and achieve remarkable curative effect.Besides,such an operation is safe and reliable,deserving to be promoted in clinical practice.
