不同部位外周静脉留置针在老年听神经瘤患者术中应用效果及安全性研究

摘　　要：目的:探讨神经外科老年听神经瘤患者开颅术中应用不同部位外周静脉留置针的效果及安全性。方法:选择首都医科大学附属北京天坛医院2017年1月—2019年10月行听神经瘤切除术的老年患者260例,按照随机数字表法将患者分为观察组和对照组,每组130例。观察组患者在下肢大隐静脉留置20G留置针,对照组患者在上肢静脉留置20G留置针。比较两组患者术中静脉留置针操作时间、一次穿刺成功率,观察术后48 h内静脉炎、局部渗液发生率及微血栓发生情况。结果:观察组静脉留置针操作时间为(3.58±0.06)min,对照组为(5.42±0.05)min,差异有统计学意义(t=2.35,P<0.05);观察组一次穿刺成功率为93.8%(122/130),对照组为81.5%(106/130),差异有统计学意义(χ^(2)=9.12,P<0.05)。置管48 h后,观察组患者发生局部渗液17例、静脉炎20例,彩色多普勒超声诊断仪发现微血栓14例,均少于对照组,但差异无统计学意义(χ^(2)值分别为1.82、2.04、0.54;P>0.05)。结论:老年听神经瘤患者术中应用下肢外周静脉导管留置综合效果优于上肢外周静脉导管,且便于临床操作,安全实用。Objective To explore the effects and safety of intraoperative application of peripheral venous indwelling needles of different parts in aged patients with acoustic neuroma.Methods Totally 260 aged patients undergoing acoustic neuroma surgery in Beijing Tiantan Hospital,Capital Medical University from January 2017 to October 2019 were selected and divided into the observation group and the control group according to the random number table,with 130 cases in each group.Patients in the observation group had a 20G indwelling needle in the saphenous vein of the lower extremities,while patients in the control group had a 20G indwelling needle in the vein of the upper extremities.The operation time of intravenous indwelling needle and the success rate of single puncture were compared between the two groups of patients.The incidence of phlebitis,local exudation and microthrombosis within 48 hours after operation was observed.Results The operation time of the intravenous indwelling needle in the observation group was(3.58±0.06)min,while that in the control group was(5.42±0.05)min,and the difference between the two groups was statistically significant(t=2.35,P<0.05).The success rate of single puncture in the observation group was 93.8%(122/130),while that in the control group was 81.5%(106/130),and the difference between the two groups was statistically significant(χ^(2)=9.12,P<0.05).At 48 hours after catheterization,17 cases of local exudation and 20 cases of phlebitis occurred in the observation group,and 14 cases of microthrombus were found by color doppler ultrasonic diagnosis apparatus,which were less than those in the control group,but the differences between the two groups were not statistically significant(χ^(2)=1.82,2.04,0.54;P>0.05).Conclusions The intraoperative application of peripheral venous indwelling needles in lower limbs in aged patients with acoustic neuroma shows better effects than that of peripheral venous catheters in upper limbs.It is clinically easy to operate,safe and practical.
