摘 要:[目的]介绍轴向应力可调节单臂外固定架治疗胫骨干骨折的手术技术与初步结果。[方法] 2015年2月~2019年1月对50例胫骨干骨折应用新型外固定支架固定调节应力治疗。通过踩踏体重秤判断应力大小,术后2周为体重的1/5, 4~6周后调至1/3体重,8~10周后2/3体重。12~14周影像检查证实骨折端有连续性骨痂,且骨折端无疼痛后可完全负重行走,X线示达到愈合标准后拆除外固定支架。[结果] 50例患者均顺利手术,手术时间(50.21±10.22) min、术中出血量(101.82±23.12) ml。随访10~12个月、骨折愈合时间(15.43±4.21)周。[结论]轴向应力可调节单臂外固定架治疗闭合性胫骨干骨折操作简单、创伤小、骨折愈合快,疗效良好。[Objective] To introduce surgical technique and preliminary clinical outcomes of a novel unilateral external fixator with adjustable axial stress for tibial shaft fractures. [Methods] From February 2015 to January 2019, 50 patients had fractures fixed with this novel unilateral external fixator with adjustable axial stress for tibial shaft fractures. After operation, the axial stress was adjusted based on stepping weight scale, with 1/5 body weight 2 weeks after operation, 1/3 body weight 4~6 weeks, and 2/3 body weight 8~10 weeks after operation. As imaging examination revealed that continuous callus was found around the fracture site about 12 to 14 weeks after operation, and no pain noted at the fracture end with full-weight-bearing, the external fixator was removed. [Results] All the patients had operation performed smoothly with operation time of(50.21±10.22) min and intraoperative blood loss of(101.82±23.12) ml. The patients were followed up for 10 to 12 months, and all of them got fracture healing in(15.43±4.21) weeks on average. [Conclusion] This novel unilateral external fixator with adjustable axial stress has benefits of simplifying surgical procedures and minimizing iatrogenic trauma, does achieve satisfactory preliminary clinical consequence for closed tibial shaft fractures.
