二氧化碳激光联合5-氨基酮戊酸光动力疗法治疗复发性尖锐湿疣的临床研究

摘　　要：目的:评价二氧化碳(CO_(2))激光联合5-氨基酮戊酸光动力疗法(ALA-PDT)治疗复发性尖锐湿疣(CA)的临床疗效和安全性。方法:将79例复发性CA患者随机分为2组,治疗组给予CO_(2)激光联合ALA-PDT治疗,对照组单独给予CO_(2)激光治疗。治疗结束后随访6个月,观察疗效和不良反应。结果:共75例患者完成治疗和随访,6个月后治疗组和对照组治愈率分别为86.8%和62.2%,差异有统计学意义(P<0.05)。其中,治疗组中单发组(疣体数量为1个者)和多发组(疣体数量超过1个者)复发率比较差异无统计学意义(P>0.05),对照组中单发组和多发组复发率比较差异有统计学意义(P<0.05)。治疗组创面愈合时间为(13.63±4.14)d,短于对照组[(16.59±4.22)d],差异有统计学意义(P<0.05)。结论:CO_(2)激光联合ALA-PDT治疗复发性CA,效果显著,能有效降低复发率,加快创面愈合,值得临床推广及应用。Objective:To investigate the clinical efficacy and safety of CO_(2) laser combined with 5-aminolevulinic acid photodynamic therapy(ALA-PDT)in the treatment of recurrent condyloma acuminatum(CA).Methods:Seventy-nine patients with recurrent CA were randomly divided into two groups(treatment group and control group).The treatment group was treated with CO_(2) laser combined with ALA-PDT,and the control group was treated with CO_(2) laser alone.After treatment,the patients were followed up for 6 months to observe the efficacy and adverse reactions.Results:A total of 75 patients completed treatment and follow-up.At 6 months,the cure rates of target lesions were 86.8%and 62.2%in the treatment group and the control group,respectively,with significant difference(P<0.05).There was no significant difference in the recurrent rate between mono-recurrence subgroup and multi-recurrence subgroup in treatment group(P>0.05),but there was significant difference in the recurrent rate between mono-recurrence subgroup and multi-recurrence subgroup in control group(P<0.05).The time of wound healing in treatment group was(13.63±4.14)d,which was significantly shorter than that in control group[(16.59±4.22)d](P<0.05).Conclusion:ALA-PDT can reduce the recurrence and shorten the time of wound healing.ALA-PDT is effective in the treatment of recurrent condyloma acuminatum after CO_(2) laser.
