缝合技能培训在骨科住院医师规范化培训中的应用和探讨

摘　　要：目的外科住院医师规范化培训是培养高素质医学人才不可缺少的阶段。针对住院医师规范化培训过程中,住院医师手术机会不多,很难作为术者独立完成基本手术等现状,探讨缝合操作技能的培训在骨科住院医师规范化培训中的应用价值。方法选取山东大学齐鲁医院新入骨科的30名住院医师进行基本缝合及肌腱缝合技能培训,基本缝合包括单纯缝合、内翻缝合等,肌腱缝合包括Kessler缝合、Bunnell缝合。30名住院医师平均分成2组,每组15名,一组培训Kesseler缝合,另一组培训Bunnell缝合。通过比较每组培训前后缝合的肌腱所能承受的最大负荷来比较培训效果。统计方法应用SPSS22.0软件配对T检验进行比较。结果基本缝合培训后,住院医师能够更加熟练地使用器械,缝合后的刀口更加美观。肌腱缝合培训后缝合的肌腱所能承受的最大负荷量明显升高(P<0.05)。结论在外科住院医师规范化培训过程中,缝合技能培训能够改善住院医师基本操作技能。因此,在住院医师规范化培训过程中需要更多、更好的培训课程用于提高基本操作技能水平,以便于取得更好的培训效果。ObjectiveTo explore the applicability of suture skill training in the process of standardized training of surgical residents.MethodsTotally thirty new surgical residents in our hospital received basic suture training, including simple interrupted suture and inverted suture, and tendon suture training involving Kessler suture and Bunnell suture. These residents were equally divided into two two groups. One group underwent Kessler suture training while and the other group received Bunnell suture training. The pre-training and post-training maximum loads that the suture tendon could bear were separately recorded and compared between the two groups by pairedsamples T Test.ResultsAfter basic suture training, the residents could use the instruments more skillfully,and the incisions after suture looked better. The post-training average maximum loads of the suture tendon in the two groups were both significantly higher(P<0. 05).ConclusionSuture skill training can be used to improve the surgical competence of residents in the process of standardized resident training. Therefore, more and better training courses are needed to improve basic surgical skills during standardized training of surgical residents so as to achieve better training effect.
