角膜绷带镜在翼状胬肉切除联合自体游离结膜瓣移植术中的应用分析

摘　　要：目的探讨角膜绷带镜在原发性翼状胬肉切除联合自体游离结膜瓣移植术中的应用效果。方法2016年5月—2018年5月方便选取在常熟市第二人民医院眼科就诊的84例(84眼)原发性翼状胬肉病例,行翼状胬肉切除联合自体游离结膜瓣移植术,随机分组为A组42例术后不配戴角膜绷带镜,B组42例术后配戴角膜绷带镜2周,对术后眼痛、异物感、流泪等症状进行评分,观察角膜上皮愈合情况,对相关数据进行统计学分析。结果配戴角膜绷带镜后患者角膜上皮愈合时间缩短[A组(4.40±0.70)d vs B组(3.09±0.05)d(t=12.097,P<0.001)],两组眼痛、异物感、流泪等症状评分术后当天A组和B组分别为(2.42±0.50)分,(1.57±0.63)分(t=6.849,P<0.001);术后1 d分别为(1.76±0.69)分,(1.14±0.61)分(t=4.363,P<0.001);术后3 d分别为(1.38±0.49)分,(0.64±0.48)分(t=6.992,P<0.001);术后7 d分别为(0.61±0.49)分,(0.37±0.49)分(t=2.245,P<0.05)。结论翼状胬肉切除联合自体游离结膜瓣移植术后配戴角膜绷带镜能够改善眼部刺激症状,促进角膜上皮修复,提高舒适度。Objective To investigate the application effect of corneal bandage mirror in primary pterygium resection combined with autologous free conjunctival flap transplantation.Methods From May 2016 to May 2018,84 cases(84 eyes)of primary pterygium who were treated in the ophthalmology department of Changshu Second People's Hospital underwent pterygium resection combined with autologous free conjunctival flap transplantation,group A of 42 cases Corneal bandages were not worn after surgery.42 cases in group B were wearing corneal bandages for 2 weeks after surgery.Postoperative eye pain,foreign body sensation,tearing and other symptoms were scored,corneal epithelial healing was observed,and related data were statistically analyzed.Results The corneal epithelium healing time was shortened after wearing a corneal bandage lens[group A(4.40±0.70)d vs group B(3.09±0.05)d(t=12.097,P<0.001)],the two groups had eye pain,foreign body sensation,and tearing Iso-symptom scores were(2.42±0.50)points and(1.57±0.63)points in group A(t=6.849,P<0.001)on the day after the operation;and(1.76±0.69)points,(1.14±0.61)points on the first postoperative day(t=4.363,P<0.001);3 days after surgery were(1.38±0.49)points,(0.64±0.48)points(t=6.992,P<0.001);7 days after surgery were respectively(0.61±0.49)points,(0.37±0.49)points(t=2.245,P=0.05).Conclusion After pterygium resection combined with free conjunctival flap transplantation,wearing a corneal bandage can improve ocular irritation,promote corneal epithelial repair,and improve comfort.
