高频彩超在胃前壁占位性病变鉴别诊断中的价值

摘　　要：目的探讨高频彩超在胃前壁占位性病变鉴别中的诊断价值。方法选择2015年9月至2018年12月于我院接受治疗的82例胃前壁占位性病变患者设为研究对象,分别对其实施胃前壁病理切片、高频彩超及胃镜检查,以病理检测结果为金标准,分析高频彩超及胃镜对胃前壁占位性病变诊断鉴别价值,而后对胃前壁占位性病变病理特征按照病情进行分析。结果(1)以病理学结果为金标准(49例为恶性病变,33例为良性病变),高频彩超对胃前壁占位性病变鉴别一致性为85.37%,灵敏度为81.63%,特异度为90.91%,胃镜鉴别一致性为86.59%,灵敏度为83.67%,特异度为90.91%,两组对比差异无统计学意义(P>0.05);(2)高频超声与病理检测结果相对比,高频超声对胃体癌、贲门癌、胃间质瘤检测符合率分别为93.33%、95.00%、100.00%,优于转移性胃癌及胃息肉检测符合率;(3)不同类型胃前壁占位性病变高频彩超具有差异性较明显的影像学特征,可用于病情鉴别。结论高频超声在胃前壁占位性病变中具有较好的诊断鉴别价值,对胃体癌、贲门癌及胃间质瘤检测的准确度较高,可通过影像学特征来鉴别胃前壁占位性病变类型。Objective To investigate the diagnostic value of high frequency color doppler ultrasound in differential diagnosis of anterior gastric wall space occupying lesions.Methods A total of 82 patients with anterior gastric wall space occupying lesions treated in our hospital from September 2015 to December 2018 were selected as the research objects.They were examined by pathological section of gastric anterior wall,high frequency color Doppler ultrasound and gastroscopy,respectively.Taking the pathological examination results as the gold standard,the diagnostic and differential value of high frequency color Doppler ultrasound and gastroscopy in the anterior gastric wall space occupying lesions were analyzed.Then,the pathological features of the anterior gastric wall space occupying lesions were analyzed according to the condition.Results(1)It was found by analysis that with the pathological results as the gold standard(49 cases were malignant lesions and 33 cases were benign lesions),the consistency,sensitivity and specificity of high frequency color Doppler ultrasound in the differential diagnosis of anterior gastric wall space occupying lesions were 85.37%,81.63%and 90.91%,respectively.The consistency,sensitivity and specificity of gastroscope identification were 86.59%,83.67%and 90.91%,respectively.There were no statistically significant differences between the two groups(P>0.05).(2)It was found by analysis that compared with pathological results,the coincidence rates of high-frequency ultrasound in detecting gastric body cancer,cardiac cancer and gastric stromal tumor were 93.33%,95.00%and 100.00%,respectively,which were superior to those of metastatic gastric cancer and gastric polyps.(3)It was found by analysis that different types of space-occupying lesions in the anterior wall of the stomach had significantly different imaging features,which could be used for disease differentiation.Conclusion High frequency ultrasound has a good diagnostic and differential value in the anterior gastric wall space occupying les
