激素替代疗法对分化型甲状腺癌术后甲状旁腺功能减退症患者的临床疗效

摘　　要：目的:观察激素替代疗法治疗分化型甲状腺癌术后甲状旁腺功能减退症患者的疗效,主要分析患者甲状旁腺激素(PTH)水平的变化,以期指导未来该类疾病的治疗方案提供选择。方法:收集2016年3月~2019年7月本院接受标准治疗方案+激素替代疗法治疗的病例52例作为观察组,回顾性分析分化型甲状腺癌术后甲状旁腺功能减退症患者病历资料;同期本院接受标准治疗方案的46例分化型甲状腺癌术后甲状旁腺功能减退症患者病历资料作为对照组。比较两组治疗前、治疗1个月、治疗3个月血钙、PTH水平;比较两组血钙、PTH恢复时间及两组治疗期间不良反应情况。结果:两组治疗1个月、治疗3个月血钙水平高于治疗前,且观察组各时点血钙水平高于对照组(P <0.05);两组治疗1个月、治疗3个月PTH水平高于治疗前,且观察组各时点PTH水平高于对照组(P <0.05);观察组血钙和PTH水平恢复时间短于对照组(P <0.05);两组恶心呕吐、便秘、心率加快和高钙尿症不良反应发生率相近。结论:激素替代疗法可有效改善分化型甲状腺术后甲状旁腺功能减退症患者血钙和PTH水平,促进患者病情恢复,且不良反应较少,安全性较高。Objective:To observe the efficacy of hormone replacement therapy in the treatment of hypoparathyroidism after differentiated thyroid cancer surgery,mainly to analyze the levels of parathyroid hormone(PTH)in patients,and guide the future treatment options for such diseases.Methods:The medical records of 52 patients with hypoparethyroidism after differentiated thyroid cancer surgery,who received standard treatment+hormone replacement therapy in our hospital from March 2016 to July 2019,were analyzed retrospectively and organized as the observation group.During the same time period,the medical records of 46 patients with hypoparathyroidism after differentiated thyroid cancer surgery who received standard treatment alone in our hospital were organized as the control group.The serum calcium(Ca)and PTH levels of the two groups before treatment,after 1 month of treatment,and 3 months of treatment were compared.The recovery time of blood Ca and PTH in the two groups was compared.The adverse reactions during the treatment of the two groups was also compared.Results:The levels of blood Ca after 1 month and 3 months of treatment in the two groups were higher than those before treatment,and the levels of blood Ca at each time point in the observation group were higher than those in the control group(P<0.05).The PTH levels after 1 month and 3 months of treatment were higher than those before treatment,and the PTH levels at each time point in the observation group were higher than those in the control group(P<0.05).The recovery time of blood Ca level and PTH level in the observation group was shorter than that in the control group(P<0.05).The incidence of adverse reactions of nausea,vomiting,constipation,increased heart rate and hypercalciuria was similar in the two groups(P>0.05).Conclusion:Hormone replacement therapy can effectively improve the serum Ca and PTH levels in patients with hypoparathyroidism after differentiated thyroid cancer surgery,which is conducive to promote the recovery of the patient’s condition,with fe
