摘 要:目的:观察平刺、直刺三阴交穴对寒凝证类痛经模型大鼠血清、子宫组织中一氧化氮(NO)及其合成酶(NOS)的影响,探讨不同刺法即刻镇痛作用的机制。方法:60只SD雌鼠随机分为空白组、模型组、直刺组、平刺组,每组15只。采用全身冷冻加雌二醇注射法制备寒凝证类痛经模型。针刺组分别直刺或平刺"三阴交"穴,留针20min。记录各组大鼠的扭体反应情况并采用超高速动态可视化微循环研究系统观察子宫微循环状况。采用生化比色法测定大鼠血清NO、NOS含量,Western blot法检测子宫组织eNOS蛋白表达。结果:与空白组比较,模型组扭体潜伏期缩短、扭体评分升高(P<0.01),微血流速度减慢(P<0.01),微血管、毛细血管(cap)管径缩小(P<0.01,P<0.05)。与模型组比较,直刺、平刺组扭体潜伏期均延长(P<0.05,P<0.01),扭体评分均显著降低(P<0.01),微血管、cap管径均显著扩张(cap管径5min时除外)(P<0.01)。平刺组针刺后15min血流状态优于直刺组(P<0.01,P<0.05),针刺后35min微血管管径较直刺组显著扩张(P<0.01)。两针刺组间扭体潜伏期及评分差异无统计学意义。与空白组比较,模型组血清NO、NOS与子宫组织eNOS蛋白表达含量显著降低(P<0.01)。与模型组比较,两针刺组血清中NO、NOS含量与子宫组织eNOS蛋白表达均升高(P<0.01,P<0.05)。平刺组NOS含量与eNOS蛋白表达均显著高于直刺组(P<0.05,P<0.01)。结论:直刺与平刺类痛经大鼠"三阴交"穴均显示出即刻镇痛效应,平刺法改善子宫微循环效果更佳,其机制可能与其更好地升高NO、NOS含量,上调子宫e NOS蛋白表达有关。Objective: To observe the effects of transverse needling and perpendicular needling at Sanyinjiao(SP6) on nitric oxide(NO) and its synthetase(nitric oxide synthase, NOS) in the serum and uterus tissue of cold congealing dysmenorrhea rats, and to explore its underlying mechanisms. Methods: A total of 60 SD female rats were randomly divided into blank group, model group, perpendicular needling group and transverse needling group, 15 rats in each group. The cold congealing dysmenorrhea rat model was prepared by systemic freezing and estradiol injection. Sanyinjiao(SP6) was needled perpendicularly or transversely in corresponding groups, and the needles were retained for 20 min. The writhing reaction and of rats in each group were recorded. The uterine microcirculation was observed by the ultra-high speed dynamic visualization microcirculation research system. Biochemical colorimetry was used to detect the levels of NO and NOS in serum of rats, eNOS expression in uterus tissues was detected by Western blot. Results: Compared with the blank group, the writhing latency was shortened and writhing score were significantly increased(P<0.01), the speed of blood flow in the microvessels slowed down(P<0.01), diameters of the uterine microvessels and capillaries(cap) shrank(P<0.01, P<0.05) in model group. Compared with the model group, the writhing latency were prolonged(P<0.05, P<0.01) and the writhing score were significantly reduced(P<0.01), and microvascular and cap diameters were significantly bigger(cap diameter at 5 minutes except)(P<0.01) in 2 acupuncture groups. Compared with perpendicular needling group, transverse needling improved the microflow state at 15 min after needling(P<0.01, P<0.05), and dilated the microvascular diameters at 35 min after needling(P<0.01). There were no statistically differences in the writhing latency and score between 2 acupuncture groups. Compared with the blank group, NO and NOS contents in the serum and eNOS expression in uterus tissues in the model group were significantly decreased
