摘  要:目的该研究旨在运用控制营养状态评分评估老年心力衰竭患者的营养状况及探讨其与常用临床指标之间的相关性。方法该研究方便选取于2018年因心衰住院的患者共224例。按照CONUT评分,将人组老年心衰患者分为3组:CONUT 0~1分即营养正常状态组,CONUT 2~4分即轻度营养不良组,CONUT逸5分即中重度营养不良组。对3组患者基线资料及临床指标进行比较。结果随着CONUT分值增加,营养状态越差的分组,淋巴细胞、血红蛋白、舒张压、心率、体质指数、总蛋白、白蛋白、甘油三酯、总胆固醇和估测肾小球滤过率越低,差异有统计学意义(P<0.05);N末端B型利納肤原(N-terminal pro-B-type natriuretic peptide,NT-proBNP)对数水平越高[(3.47±0.39)Pg/mLV vs(3.54±0.39)Pg/mLV vs(3.82±0.34)Pg/mL],差异有统计学意义(P<0.05)。经Pearson相关性分析显示:年龄、NT-proBNP对数水平与CONUT评分呈正相关,差异有统计学意义(P<0.05)。此外,体质指数、血红蛋白、甘油三酯、高密度脂蛋白、低密度脂蛋白和估测肾小球滤过率与CONUT评分呈负相关,差异有统计学意义(P<0.05)。结论老年心力衰竭患者随着CONUT分值增加,营养状态越差。CONUT评分与血红蛋白、估测的肾小球滤过率和血浆利钠肽水平等临床指标存在相关性。Objective This study aims to use the control nutritional status score to assess the nutritional status of elderly patients with heart failure and explore its correlation with common clinical indicators.Methods This study conveniently selected 224 patients hospitalized with heart failure in 2018.According to the CONUT score,the enrolled elderly patients with heart failure were divided into 3 groups:CONUT 0-1 is the normal nutritional status group,CONUT 2-4 is the mild malnutrition group,and CONUT≥5 is the moderate to severe malnutrition group.The baseline data and clinical indicators of the three groups of patients were compared.Results This study showed that as the CONUT score increases,the worse the nutritional status is,the lower the lymphocytes,hemoglobin,diastolic blood pressure,heart rate,body mass index,total protein,albumin,triglycerides,total cholesterol and estimated glomerulus filtration rate,the difference was statistically significant(PcD.05);the higher the logarithmic level of N-terminal pro-B-type natriuretic peptide(NT-proBNP)[(3.47±0.39)Pg/mLV vs(3.54±0.39)Pg/mLV vs(3.82±0.34)Pg/mL],there was a statistically significant difference(P<0.05).Pearson correlation analysis showed that age,NT-proBNP logarithmic level and CONUT score were positively correlated with statistically significant differences(P<0.05).In addition,body mass index,hemoglobin,triglycerides,high-density lipoprotein,low-density lipoprotein,and estimated glomerular filtration rate were negatively correlated with CONUT scores,and there were statistically significant differences(P<0.05).Conclusion The nutritional status of elderly patients with heart failure increases as the CONUT score increases.The CONUT score is correlated with clinical indicators such as hemoglobin,estimated glomerular filtration rate and plasma natriuretic peptide levels.
