胰岛素联合湿性敷料在糖尿病足溃疡生长期治疗的效果观察

摘　　要：目的:观察胰岛素联合湿性敷料在糖尿病足溃疡生长期治疗的效果。方法:选取2019年1月—2020年6月在我院就诊的糖尿病足处于溃疡肉芽生长期的患者60例,采用随机数字表法分为实验组32例,对照组28例。对照组采取藻酸盐敷料方案,实验组在对照组的基础上加用胰岛素进行创面护理,比较两组的临床疗效及创面面积变化。结果:实验组治疗后治愈21例、有效9例、无效2例,总有效率为93.75%,对照组治愈14例、有效8例、无效6例,总有效率为75.00%,差异有统计学意义(P<0.05);实验组治疗后4周创面面积为(4.65±1.26)cm2,对照组为(5.03±1.69)cm2,实验组均显著低于对照组(P<0.05)。结论:胰岛素联合湿性敷料在糖尿病足溃疡生长期的治疗中能够促进伤口的愈合,减少溃疡面创面的面积,减轻患者的痛苦,改善患者的生活质量。Objective To investigate the therapeutic effect of insulin combined with wet dressing in diabetic foot ulcer growth stage.Methods A total of 60 patients with diabetic foot in the ulcerative granulation stage who visited our hospital from January 2019 to June 2020 were selected as research objects,and were divided into experimental group(32 cases)and control group(28 cases)by random number table method.To comparing the clinical efficacy and wound area change,the control group adopted the wet dressing(alginate dressing)and the experimental group added insulin on the basis of the control group for wound nursing.Results After treatment,21 cases were cured in the experimental group,9 cases were effective and 2 cases were ineffective,with a total effective rate of 93.75%,while 14 cases were cured in the control group,8 cases were effective and 6 cases were ineffective,with a total effective rate of 75.00%,the difference was statistically significant(P<0.05);4 weeks after treatment,the wound area of the experimental group was(4.65±1.26)cm2,and that of the control group was(5.03±1.69)cm2,which was significantly lower than the control group(P<0.05).Conclusion Insulin combined with wet dressing in the treatment of diabetic foot can promote wound healing,effectively reduce the time of ulcer healing,relieve the pain of patients,and improve the quality of life of patients.
