ERAS在胸腰椎后凸畸形截骨矫形治疗中的应用效果

摘　　要：目的:了解加速康复外科(E R A S)在胸腰椎后凸畸形截骨矫形治疗中的应用效果。方法:将2017年1月—2019年1月在我院脊柱外科完成的15例胸腰段脊柱后凸畸形VCR截骨矫形术的患者纳入实验组(ERAS组);另将采用常规手术管理的21例胸腰段脊柱后凸畸形VCR截骨矫形术的患者纳入对照组(非ERAS组)。对比观察两组的最大后凸Cobb角(global kyphosis,GK)以及术后Cobb角;疼痛视觉模拟评分(visual analog scale,VAS);Oswestry功能障碍(ODI)。手术时间;术中失血量;术后引流量;并发症情况。结果:实验组的手术时间(225±48)min,术中失血量(550±80)ml,术后引流量(350±52)ml。对照组手术时间(290±89)min,术中失血量(870±120)ml,术后引流量(580±72)ml。两组比较均有统计学差异(P<0.05)。手术前后Cobb角、VAS评分、ODI评分比较实验组均优于对照组,差异有统计学意义(P<0.05)。结论:在截骨矫形手术中,E R A S的应用确实能加速病员的康复,具有良好的社会效益及经济效益,值得应用。Objective To observe the effect of accelerated rehabilitation surgery(ERAS)in the treatment of thoracolumbar kyphosis.Method A total of 15 patients with thoracolumbar kyphosis who underwent VCR osteotomy and orthopedic surgery in the spine department of our hospital from January 2017 to January 2019 were included in the experimental group(ERAS group).A total of 21 patients with thoracolumbar kyphosis treated with VCR osteotomy and orthopedic surgery were included in the control group(non-ERAS group).The maximum kyphotic Cobb Angle(global kyphosis,GK)and postoperative Cobb Angle of the two groups were compared and observed.Visual Analog Scale(VAS);Oswestry Disability(ODI).Operation time;Intraoperative blood loss;Postoperative drainage volume;Complications.Results The operation time(225±48/290±89min),blood loss(550±80/870±120ml)and postoperative drainage volume(350±52/580±72ml)of the experimental group were less than those of the control group,The difference was statistically significant(P﹤0.05).The postoperative Cobb angle,VAS,ODI,and complications of the ERAS group were better than those of the non-ERAS group,The difference was statistically significant(P﹤0.05).Conclusion For osteotomy,the application of ERAS can accelerate the recovery of patients,and has good social and economic benefits,worthy of application.
