摘　　要：目的探讨课题研究型品管圈在构建多维度临床路径管理新模式中的应用。方法成立“质管圈”及圈外专家咨询团,依据课题研究型品管圈操作步骤,从主管部门、临床科室、医技和医疗辅助科室3个层面分析现状,明确6大攻坚点,多方拟定对策并加以筛选判断,从建设规范化管理制度体系、信息化管理平台、“线上+线下”混合式培训模式和动态质量监管体系等方面改进。结果构建了多维度临床路径管理新模式,使临床路径入径率、完成率、覆盖率分别从46.5%、70.3%、30.3%提高至61.1%、81.3%、52.0%,同时缩短了平均住院日;总体指标、效率指标实施前后比较差异具有统计学意义(P<0.05)。结论课题研究型品管圈活动可有效推进临床路径多维度管理模式的构建,对医疗机构规范临床诊疗行为,提高医疗质量,有积极促进作用。Objective To explore the application of the task achieving quality control circle in the construction of the new multi-dimensional clinical route management model. Methods Establish the quality control circle and the experts advisory out of the circle;analyze current situation of administrative department, clinical departments and the medical detection department;define and make effort to solve the 6 difficult points in multiple aspects according to the operational procedure;make improvements by developing a standardized management system, an informationizedmanagement platform, an online-offline combined training pattern and a dynamic quality supervising system. Results The new model of multidimensional clinical pathway management was constructed, which increased the access rate, completion rate and coverage rate of clinical pathway from 46.5%, 70.3% and 30.3% to 61.1%, 81.3% and 52.0% respectively, and shortened the average length of stay at the same time. The differences of overall index and efficiency index before and after implementation were statistically significant(P < 0.05). Conclusion Research oriented QCC activities can effectively promote the construction of multi-dimensional management modelof clinical pathway, and play a positive role in standardizing clinical diagnosis and treatment behavior and improving medical quality of medical institutions.
