腺样体肥大引起分泌性中耳炎的原因、程度及手术疗效分析

摘　　要：目的分析腺样体肥大引起分泌性中耳炎(SOM)的原因、程度与手术疗效,以期为后期腺样体肥大、SOM治疗工作及相关研究提供参考。方法方便选择该院2018年10月-2019年10月收治的58例腺样体肥大患儿,均施以听力检查,对其中36例腺样体肥大合并SOM患儿实施腺样体刮除术治疗,22例单纯腺样体肥大患儿不予以任何治疗;观察36例腺样体肥大合并SOM患儿手术治疗效果、22例单纯腺样体肥大患儿病情变化。结果 36例腺样体肥大合并SOM患儿手术治疗优良率为94.74%。22例单纯腺样体肥大患儿SOM发病率为27.27%。结论腺样体肥大引起SOM的主要机理为咽鼓管机械压迫堵塞、腺样体内细菌、咽鼓管反流、腺样体免疫功能不良。早期鉴别腺样体肥大合并SOM,并施以手术治疗,可获取理想治疗效果。Objective To analyze the cause,degree and surgical effect of secretory otitis media(SOM)caused by adenoid hypertrophy,in order to provide reference for later adenoid hypertrophy,SOM treatment and related research.Methods 58 children with adenoid hypertrophy who were admitted to the hospital from October 2018 to October 2019 were conveniently selected,and all of them were subjected to hearing examination,and 36 children with adenoid hypertrophy and SOM were subjected to adenoid curettage,22 children with simple adenoid hypertrophy were not given any treatment;36 children with adenoid hypertrophy combined with SOM were observed for surgical treatment,and 22 children with simple adenoid hypertrophy were observed.Results The excellent and good rate of surgical treatment of 36 children with adenoid hypertrophy and SOM was 94.74%.The incidence of SOM in 22 children with simple adenoid hypertrophy was 27.27%.Conclusion The main mechanism of adenoid hypertrophy caused SOM is mechanical compression and blockage of the Eustachian tube,bacteria in the adenoids,reflux of the Eustachian tube,and poor immune function of the adenoids.Early identification of adenoid hypertrophy combined with SOM and surgical treatment can obtain ideal therapeutic effects.
