摘　　要：目的探究Quadrant可扩张通道下微创经椎间孔入路腰椎椎体间融合术(MIS-TLIF)治疗单节段退变性腰椎管狭窄症患者的近远期效果。方法以2015年12月至2019年9月收治的84例单节段退变性腰椎管狭窄症患者为研究对象,根据简单随机化法将其分为对照组(n=42)和试验组(n=42)。对照组实施经椎间孔入路腰椎椎体间融合术(TLIF)治疗,试验组实施Quadrant可扩张通道下MIS-TLIF术治疗。比较两组患者的治疗效果。结果试验组患者的手术用时、切口长度、术后下床时间、住院时间均短于对照组,术中出血量、术后引流量均少于对照组(P<0.05)。术后6个月,两组的ODI、VAS评分均低于术前,JOA评分高于术前,且试验组均优于对照组(P<0.05)。试验组的并发症总发生率为4.76%,低于对照组的21.43%(P<0.05)。结论Quadrant可扩张通道下MIS-TLIF术治疗单节段退变性腰椎管狭窄症患者,能够缩短手术时间和住院时间,提高治疗效果,减少并发症。Objective To investigate the short-term and long-term efficacy of minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)with Quadrant expandable channel in the treatment of patients with single segment degenerative lumbar spinal stenosis.Methods A total of 84 patients with single segment degenerative lumbar spinal stenosis admitted from December 2015 to September 2019 were selected as the research objects,and the patients were divided into control group(n=42)and experimental group(n=42)according to the simple randomized method.The control group was treated with transforaminal lumbar interbody fusion(TLIF),and the experimental group was treated with MIS-TLIF surgery with Quadrant expandable channel.The therapeutic effects of the two groups were compared.Results The operation time,incision length,postoperative ambulation time and hospitalization time of the experimental group were shorter than those of the control group,the intraoperative blood loss volume and postoperative drainage volume were less than those of the control group(P<0.05).At 6 months after operation,the ODI and VAS scores of the two groups were lower than those before operation,the JOA scores were higher than those before operation,and those of the experimental group were better than the control group(P<0.05).The total incidence of complications in the experimental group was 4.76%,which was lower than 21.43%in the control group(P<0.05).Conclusion MIS-TLIF surgery with Quadrant expandable channel in the treatment of patients with single segment degenerative lumbar spinal stenosis can shorten the operation time and hospitalization time,improve the treatment effect and reduce complications.
